Comparison of effects of minimally invasive percutaneous plate internal fixation and subcutaneous internal fixator in treatment of patients with unstable anterior pelvic ring fractures
Objective:To compare effects of minimally invasive percutaneous plate internal fixation and subcutaneous internal fixator in treatment of patients with unstable anterior pelvic ring fractures.Methods:The clinical data of 106 patients with unstable anterior pelvic ring fractures admitted to this hospital from June 2020 to June 2022 were retrospectively analyzed.According to different surgical methods,they were divided into observation group(n=53)and control group(n=53).The control group was treated with subcutaneous internal fixator,while the observation group was treated with minimally invasive percutaneous plate internal fixation.The levels of operation-related indicators(hospitalization time,operation time,fracture healing time,intraoperative blood loss)and serum bone transformation status indicators[bone-specific alkaline phosphatase(BAP),osteocalcin(OCN),total type Ⅰ collagen amino terminal peptide(T-P1NP)],the pelvic vertical displacement distance,and the incidence of complications were compared between the two groups.Results:The hospitalization time,operation time,and fracture healing time in the observation group were shorter than those in the control group,and intraoperative blood loss in the observation group was less than that in the control group,and the differences were statistically significant(P<0.05).2 weeks after the surgery,the levels of serum BAP and OCN in the two groups were higher than those before the surgery,and those in the observation group were higher than those in the control group;the serum T-P1NP level in the two groups was lower than that before the surgery,and that in the observation group was lower than that in the control group;and the differences were statistically significant(P<0.05).6 months after the surgery,the pelvic vertical displacement distance levels in the two groups were lower than those before the surgery,that in the observation group was lower than that in the control group,and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusions:Minimally invasive percutaneous plate internal fixation in the treatment of the patients with unstable anterior pelvic ring fractures can improve the levels of bone transformation status indicators,reduce the levels of surgery-related indicators and the pelvic vertical displacement distance.Moreover,it is superior to subcutaneous internal fixator.
