Effects of coronary artery bypass grafting combined with cardiac valve replacement surgery in treatment of patients with coronary heart disease complicated with valvular heart disease
Objective:To observe effects of coronary artery bypass grafting combined with cardiac valve replacement surgery in treatment of patients with coronary heart disease complicated with valvular heart disease.Methods:A prospective study was conducted on 80 patients with coronary heart disease and valvular heart disease admitted to the hospital from March 2019 to June 2021.According to the random number table method,they were divided into study group and control group,40 cases in each group.The control group was treated with coronary artery bypass grafting combined with non-simultaneous cardiac valve replacement surgery,while the study group was treated with simultaneous coronary artery bypass grafting combined with cardiac valve replacement surgery.The clinical efficacy,the levels of cardiac function indexes[left ventricular ejection fraction(LVEF),left ventricular end-diastolic diameter(LVEDD),cardiothoracic ratio]and myocardial injury markers[creatine kinase isoenzyme(CK-MB),growth differentiation factor-15(GDF-15),N-terminal pro-brain natriuretic peptide(NT-proBNP)]before and after the surgery,and the incidence of adverse cardiovascular events were compared between the two groups.Results:The total effective rate of the study group was 92.50%(37/40),which was higher than 75.00%(30/40)of the control group,and the difference was statistically significant(P<0.05).At 8 months after the surgery,the LVEF levels of the two groups were higher than those before the surgery,and that in the study group was higher than that in the control group;the LVEDD and the cardiothoracic ratio of the two groups were lower than those before the surgery,and those in the study group were lower than those in the control group;and the differences were statistically significant(P<0.05).At 8 months after the surgery,the levels of GDF-15,CK-MB and NT-proBNP in the two groups were lower than those before the surgery,and those in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).Further,the incidence of adverse cardiovascular events in the study group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusions:Coronary artery bypass grafting combined with cardiac valve replacement surgery for the patients with coronary heart disease and valvular heart disease can improve the total effective rate of treatment,improve the levels of cardiac function indexes,and reduce the level of myocardial injury markers and the incidence of adverse cardiovascular events.Moreover,it is superior to coronary artery bypass grafting combined with non-simultaneous cardiac valve replacement surgery.
