Effects of double incision double plate internal fixation in treatment of patients with complex tibial plateau fractures
Objective:To observe effects of double incision and double plate internal fixation in treatment of patients with complex tibial plateau fractures.Methods:A prospective study was conducted on 85 patients with complex tibial plateau fractures admitted to this hospital from October 2021 to June 2023.They were divided into study group(n=43)and control group(n=42)according to the random number table method.The control group was treated with unilateral locking compression plate internal fixation,while the study group was treated with double incision and double plate internal fixation.The excellent and good rate of the surgery,the joint function[Lysholm knee scale(LKS)]scores before and after the surgery,the tibial plateau index(posterior slope angle,varus angle,axial stiffness)levels,and the incidence of complications were compared between the two groups.Results:The excellent and good rate of the surgery in the study group was 95.35%(41/43),which was higher than 78.57%(33/42)in the control group,and the difference was statistically significant(P＜0.05).At 3 months after the surgery,the LKS scores of the two groups were higher than those before the surgery,that in the study group was higher than that in the control group,and the differences were statistically significant(P＜0.05).The posterior slope angle and the varus angle of tibial plateau in the two groups were lower than those before the surgery,and those in the study group were lower than those in the control group;the axial stiffness of the study group was higher than that of the control group;and the differences were statistically significant(P＜0.05).Further,the incidence of complications in the study group was 6.98%(3/43),which was lower than 23.81%(10/42)in the control group,and the difference was statistically significant(P＜0.05).Conclusions:Double incision double plate internal fixation for the treatment of the patients with complex tibial plateau fractures can improve the excellent and good rate of surgery and the knee function,improve the posterior slope angle,varus angle and axial stiffness of the tibial plateau,and reduce the incidence of complications.Moreover,it is superior to unilateral locking compression plate internal fixation.
