Value of three-dimensional hysterosalpingo-contrast sonography in diagnosis of tubal patency in infertile patients
Objective:To observe value of three-dimensional hysterosalpingo-contrast sonography in diagnosis of tubal patency in infertile patients.Methods:A prospective study was conducted on 150 patients with infertility admitted to the hospital from June 2021 to June 2023.All patients underwent three-dimensional hysterosalpingo-contrast sonography,X-ray hysterosalpingography and laparoscopic methylene blue hydrotubation.Taking the results of laparoscopic methylene blue hydrotubation as the gold standard,the efficiencies of three-dimensional hysterosalpingo-contrast sonography and X-ray hysterosalpingography in the diagnosis of fallopian tube obstruction in the infertile patients was compared.Kappa consistency test was used to analyze the consistency of three-dimensional hysterosalpingo-contrast sonography,X-ray hysterosalpingography and laparoscopic methylene blue hydrotubation in the diagnosis of fallopian tube obstruction.Results:The results of laparoscopic methylene blue hydrotubation showed that among the 300 fallopian tubes of the 150 infertility patients,202 fallopian tubes were obstructed and 98 fallopian tubes were unobstructed.The sensitivity,the accuracy and the negative predictive value of three-dimensional hysterosalpingo-contrast sonography in the diagnosis of fallopian tube obstruction were higher than those of X-ray hysterosalpingography,the rate of missed diagnosis was lower than that of X-ray hysterosalpingography,and the differences were statistically significant(P<0.05).The consistency of transvaginal three-dimensional hysterosalpingography and laparoscopic methylene blue hydrotubation(Kappa=0.878)was higher than that of X-ray hysterosalpingography and laparoscopic methylene blue hydrotubation in the diagnosis of fallopian tube obstruction(Kappa=0.698).Conclusions:The sensitivity,the accuracy and the negative predictive value of three-dimensional hysterosalpingo-contrast sonography as well as its diagnostic consistency with laparoscopic methylene blue hydrotubation in the diagnosis of fallopian tube obstruction are higher than those of X-ray hysterosalpingography,and the rate of missed diagnosis is lower than that of X-ray hysterosalpingography.
