Comparison of effects of PHILOS titanium plate internal fixation and intramedullary nail fixation in treatment of elderly patients with comminuted proximal humeral fractures
Objective:To compare effects of proximal humeral internal locking system(PHILOS)titanium plate internal fixation and intramedullary nail fixation in treatment of elderly patients with comminuted proximal humeral fractures.Methods:A prospective study was conducted on 60 elderly patients with comminuted proximal humeral fractures admitted to this hospital from June 2021 to December 2022.According to the random number table method,they were divided into observation group and control group,30 cases in each group.The control group was treated with intramedullary nail fixation,while the observation group was treated with PHILOS titanium plate internal fixation.The excellent and good rate of operation,the Neer score,the operation time,the fracture healing time,and the incidence of complications were compared between the two groups.Results:The excellent and good rate of operation in the observation group was 93.33%,which was significantly higher than 70.00%in the control group,and the difference was statistically significant(P<0.05).After the treatment,the Neer scores of the two groups were higher than those before the treatment,that in the observation group was higher than that in the control group,and the differences were statistically significant(P<0.05).The fracture healing time of the observation group was shorter than that of the control group,and the difference was statistically significant(P<0.05).The incidence of complications in the observation group was 6.67%,which was significantly lower than 26.67%in the control group,and the difference was statistically significant(P<0.05).However,there was no significant difference in operation time between the two groups(P>0.05).Conclusions:PHILOS titanium plate internal fixation in the treatment of the elderly patients with comminuted proximal humeral fractures can improve the excellent and good rate of surgery and the postoperative Neer scores,shorten the fracture healing time,and reduce the incidence of complications.Moreover,it is superior to intramedullary nail fixation.
