Application effects of spiral suture of lower uterine segment in cesarean section patients with pernicious placenta previa
Objective:To observe application effects of spiral suture of lower uterine segment in patients with pernicious placenta previa(PPP)undergoing cesarean section.Methods:A prospective study was conducted on 68 PPP patients admitted to the hospital from January 2022 to October 2023.According to the random number table method,they were divided into observation group and control group,34 cases in each group.Both groups underwent cesarean section.The control group was treated with B-Lynch suture combined with gauze packing in the lower uterine segment for hemostasis,while the observation group was treated with spiral suture in the lower uterine segment for hemostasis.The hemostatic effects,the perioperative indexes(operation time,hospitalization time,bleeding volume 24 h after the surgery),the hemoglobin(Hb)levels before and after the surgery,the hysterectomy rate,the uterine artery embolization rate,and the complication rate were compared between the two groups.Results:The total effective rate of hemostasis in the observation group was 97.06%(33/34),which was higher than 76.47%(26/34)in the control group,and the difference was statistically significant(P<0.05).The operation time and the hospitalization time of the observation group were shorter than those of the control group,the bleeding volume 24 h after the surgery was less than that of the control group,and the differences were statistically significant(P<0.05).3 days after the surgery,the Hb level in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).There were no significant differences in the hysterectomy rate and the uterine artery embolization between the two groups(P>0.05).The complication rate in the observation group was 5.88%(2/34),which was lower than 26.47%(9/34)in the control group,and the difference was statistically significant(P<0.05).Conclusions:The spiral suture in the lower uterine segment for hemostasis in the patients with PPP undergoing cesarean section can improve the hemostatic effects,improve the perioperative index levels and the Hb levels,and reduce the complication rate.Moreover,it is superior to B-Lynch suture combined with lower uterine segment gauze packing.
