Comparison of application effects of transannular patch and preserved valve ring patch in children with tetralogy of Fallot correction surgery
Objective:To compare application effects of transannular patch and preserved valve ring patch in children with tetralogy of Fallot correction surgery. Methods:The clinical data of 100 children with tetralogy of Fallot admitted to this hospital from April 2021 to May 2023 were retrospectively analyzed. According to the different ways of relieving the stenosis of right ventricular outflow tract,they were divided into observation group and control group,50 cases in each group. Both groups underwent correction surgery of tetralogy of Fallot. The observation group was treated with transannular patch,while the control group was treated with preserved valve ring patch. The pulmonary transvalvular gradient (PTG),the arterial oxygen saturation (SaO2),the pulmonary artery development index (pulmonary valve ring diameter,Nakata index,McGoon ratio) levels,the cardiac structural parameters[right ventricular end-diastolic diameter index (RVEDDI),left ventricular end-diastolic volume index (LVEDVI),left ventricular end-diastolic diameter index (LVEDDI)]levels,and the postoperative reintervention rate were compared between the two groups before and after the surgery. Results:At 6 months after the surgery,the PTG levels of the two groups were lower than those before the surgery,and that in the observation group was lower than that in the control group;the SaO2 levels of the two groups were higher than those before the surgery,and that in the observation group was higher than that in the control group;and the differences were statistically significant (P>0.05). At 6 months after the surgery,the levels of Nakata index,McGoon ratio and pulmonary valve ring diameter of the two groups were higher than those before the surgery,those in the observation group were higher than those in the control group,and the differences were statistically significant (P<0.05). At 12 months after the surgery,the levels of RVEDDI and LVEDVI in the two groups were higher than those before the surgery,and those in the observation group were higher than those in the control group;the LVEDDI levels of the two groups were lower than those before the surgery,and that in the observation group was lower than that in the control group;and the differences were statistically significant (P<0.05). However,there was no significant difference in the re-intervention rate between the two groups. Conclusions:The transannular patch in the children with tetralogy of Fallot can improve the levels of SaO2,the levels of pulmonary artery development indexes,improve the levels of cardiac structural parameters,and reduce the levels of PTG. Moreover,it is superior to preserved valve ring patch.
