Effects of fiberoptic bronchoscopy alveolar lavage combined with Azithromycin in treatment of children with lobar pneumonia
Objective:To observe effects of fiberoptic bronchoscopy alveolar lavage combined with Azithromycin in treatment of children with lobar pneumonia. Methods:A prospective study was conducted on 64 children with lobar pneumonia admitted to this hospital from 2021 to 2023. According to the random number table method,they were divided into control group and observation group,32 cases in each group. The control group was treated with Azithromycin,while the observation group was treated with fiberoptic bronchoscopy alveolar lavage on the basis of that of the control group. The clinical efficacy,the symptom disappearance time (lung rales,lung shadows,high fever,cough),the levels of serum inflammatory factors[interleukin-6 (IL-6),tumor necrosis factor-α (TNF-α),C-reactive protein (CRP)],the lung function indicators[peak expiratory flow (PEF),forced vital capacity (FVC),forced expiratory volume in one second (FEV1)],and the immunoglobulin index[immunoglobulin A (IgA),immunoglobulin G (IgG),immunoglobulin M (IgM)]and were compared between the two groups. Results:The total effective rate of the observation group was 96.87% (31/32),which was higher than 75.00% (24/32) of the control group,and the difference was statistically significant (P<0.05). The disappearance time of lung rales,lung shadows,high fever and cough in the observation group was shorter than that in the control group,and the difference was statistically significant (P<0.05). After the treatment,the levels of CRP,TNF-α and IL-6 in the two groups were lower than those before the treatment,those in the observation group were lower than those in the control group,and the differences were statistically significant (P<0.05). After the treatment,the levels of FEV1,FVC and PEF in the two groups were higher than those before the treatment,those in the observation group were higher than those in the control group,and the differences were statistically significant (P<0.05). Further,after the treatment,the levels of IgA,IgG and IgM in the two groups were higher than those before the treatment,those in the observation group were higher than those in the control group,and the differences were statistically significant (P<0.05). Conclusions:Fiberoptic bronchoscopy alveolar lavage combined with Azithromycin in the treatment of the children with lobar pneumonia can improve the total effective rate of treatment,the lung function index and the immunoglobulin index levels,shorten the symptom disappearance time,and reduce the inflammatory response. Moreover,it is superior to simple Azithromycin treatment.
