Predictive value of carotid plaque ultrasound features for ischemic stroke in high-risk population of stroke
Objective:To observe Predictive value of carotid plaque ultrasound features for ischemic stroke in high-risk population of stroke. Methods:A prospective study was conducted on 125 patients with carotid plaque who met the criteria of high-risk population of stroke from May 2020 to December 2022. After 1 year of follow-up,the incidence of ischemic stroke was observed. They were divided into occurrence group and non-occurrence group according to whether ischemic stroke occurred. The ultrasonic features of carotid plaque were compared between the two groups. Multivariate Logistic regression analysis was used to analyze the influencing factors of ischemic stroke. The receiver operating characteristic (ROC) curve used to analyze the predictive value of carotid plaque ultrasound features for ischemic stroke in the high-risk population of stroke. Results:After 1 year of follow-up,86 cases of ischemic stroke occurred in the 125 patients with carotid plaque who met the criteria of high-risk population of stroke,with an incidence of 68.80% (86/125). The plaque thickness,the proportion of irregular or ulcerative plaques,the proportion of vulnerable plaques,and the proportion of moderate to severe stenosis in the occurrence group were higher than those in the non-occurrence group,and the differences were statistically significant ( P<0.05). Logistic regression analysis showed that vulnerable plaque and severe carotid stenosis were risk factors for ischemic stroke in the patients with carotid plaque who met the criteria of high-risk population of stroke (OR>1,P<0.05). ROC curve analysis showed that the predictive value of carotid stenosis degree combined with plaque stability for ischemic stroke in the patients with carotid plaque who met the criteria of high-risk population of stroke[area under the curve (AUC)=0.934]was higher than the single predictive value of the two (AUC=0.905,0.702). Conclusions:The predictive value of carotid stenosis degree combined with plaque stability for ischemic stroke in the patients with carotid plaque who meet the criteria of high-risk population of stroke is higher than the single predictive value of the two.
