Effect of hospital-community-family linkage intervention on elderly patients with hypertension multimorbidity
Objective To explore the effect of hospital-community-family linkage intervention on elderly patients with hypertension multimorbidity.Methods The elderly patients with hypertension multimorbidity hospi-talized in the Cardiology Department of a tertiary A hospital in Datong City from January to June 2023 were recrui-ted in this study.The subjects were assigned into intervention group and control group by random number table method.Patients in the control group received conventional rehabilitation intervention,while those in the interven-tion group received hospital-community-family linkage intervention on the basis of conventional intervention.The self-management behavior,blood pressure,self-efficacy and family function scores were compared between the two groups before and 2 months after intervention.Results After 2-month intervention,the scores of self-management behavior,general self-efficacy and family function in the intervention group were significantly higher than those in the control group(t=5.683,P<0.001;t=3.428,P<0.01;t=10.460,P<0.001);The systolic blood pres-sure(SBP)(t=3.519,P<0.001)and diastolic blood pressure(DBP)(t=2.056,P<0.05)in the interven-tion group exhibited a statistically significant reduction compared to those in the control group.Conclusions Hos-pital-community-family linkage intervention can significantly reduce the blood pressure of elderly patients with hy-pertension multimorbidity,enhance their self-management ability,self-efficacy and family function,so as to im-prove their quality of life.
