Status quo of family bed service in Jinshan District of Shanghai from 2021 to 2023
Objective To analyze the status quo of hospital-at-home care and to provide reference for op-timizing the work.Methods The data of hospital-at-home care carried out by 11 community health service cen-ters in Jinshan District during 2021-2023 were extracted from the database of Shanghai Jinshan District Health In-formation System,and retrospective survey was performed for the collection and analysis of the patients'gender,age,marital status,occupation,residence,type of care,expense classification,category of disease and service i-tems.Results The total number of home sickbeds in Jinshan District of Shanghai was 5 358 from 2021 to 2023.The annual number of sickbeds,the total income of hospital-at-home care and the average cost for hospital-at-home care all showed an increasing trend year by year.The majority of patients were over 80 years old,with an average age of(75.02±13.36)years,which decreased year by year.There was no significant difference in hospital-at-home care among different years and patients with different gender(P＞0.05).There were statistically significant differences among patients with different age,marital status,occupation,residence,type of care and expense clas-sification in different years.The proportion of patients with age ≥80 years old(2021),married(2023),retire-ment(2021),town residence(2021),western medical care(2021),and medical insurance expense(2023)was higher than that of other corresponding groups(P＜0.05).The number of home sickbeds per 100 people in the community with the highest number was 2.39 times that in the community with the lowest number.Patients of hospital-at-home care had all kinds of diseases,which were mainly common chronic diseases and COVID-19.The top three service items were home sickbed visit,blood/urine/stool test,and dressing change.Conclusion Hos-pital-at-home care in Jinshan District has gradually produced good effects,but the promotion of the care and the energy efficiency of the service still need to be improved.It is also necessary to strengthen the publicity of hospital-at-home care policy,to reduce the related costs by the overall planning of the government,to expand service pro-jects,to provide precise services for suitable patients,to enhance the training and quality management of medical staff,and to constantly improve the content and quality of hospital-at-home care.
