经尿道钬激光前列腺中叶剜除术治疗前列腺中叶增生的中年患者的可行性研究

摘要
目的 探讨经尿道钬激光前列腺中叶剜除术在低龄且以中叶增生为主的前列腺增生患者中应用.方法 选择2022年1月至2022年6月于青岛大学附属医院泌尿外科以前列腺中叶增生入院,且有性生活需求的20例患者,术前评估前列腺突入膀胱程度(intravesical prostatic protrusion,IPP)均大于1.5 cm,行经尿道钬激光前列腺中叶剜除术,对其手术前后排尿症状、残余尿量、国际前列腺症状评分(international prostate symptom score,IPSS)、尿流率、尿失禁及勃起功能评分(International Index of Erectile Function-5,IIEF-5)、射精情况进行分析.结果 患者排尿困难症状均明显改善,无尿失禁患者,术后1年回访,逆向射精患者1例,逆向射精发生率5％.结论 以中叶增生为主的前列腺增生患者行单纯经尿道钬激光前列腺中叶剜除术,围手术期各项指标改善明显,具有明显的治疗效果,可减少逆向射精、尿失禁、膀胱颈挛缩的发生率.
Abstract
Objective To investigate the application of transurethral holmium laser enucleation of the middle lobe of the prostate in middle-aged patients with middle lobe hyperplasia.Methods A total of 20 cases of prostatic hyperplasia with middle lobe hyperplasia and sexual needs admitted to the department of urology,affiliated hospital of Qingdao university from January 2022 to June 2022 were enrolled in the study.Intravesical prostatic protrusion(IPP)of prostatic protrusion into the bladder was more than 1.5 cm before surgery.Urinary symptoms,residual urine volume,international prostate symptom score(IPSS),urinary flow rate,urinary incontinence,International Index of Erectile Function-5(IIEF-5)and ejaculation were analyzed after tranthral holmium laser enucleation of the middle lobe of the prostate.Results The symptoms of dysuria were significantly improved in all patients,and no incontinence patient was observed.There was one case of reverse ejaculation 1 year after surgery,and the incidence of reverse ejaculation was 5％.Conclusion Simple transurethral holmium laser enucleation of the middle lobe of the prostate for the patients with middle lobe hyperplasia significantly improves perioperative indexes,and has prominent therapeutic effect,which can reduce the incidences of reverse ejaculation,urinary incontinence and bladder neck contracture.
