目的 评价增厚避孕套在治疗早泄中的作用.方法 选取2021 年8 月至12 月在浙江大学医学院附属第一医院泌尿外科男科中心治疗的100 例早泄患者作为早泄组,30 例正常男性作为正常组.测定早泄组和正常组使用普通与增厚避孕套后的安全性、舒适度、阴茎背神经传导速度、射精潜伏期、阴茎头敏感性、阴茎勃起功能和血清生殖激素水平.结果 早泄患者使用增厚避孕套后,相较普通避孕套,舒适度降低(80/100 对比 60/100,P<0.05);阴茎背神经传导速度减慢[(30.5±2.7)m/s vs(21.04±1.9)m/s,P<0.05];阴茎振动阈值显著升高[(29±2.5)级 vs(47±2.0)级,P<0.05];射精潜伏期(>3 min)(16/100 vs 78/100,P<0.05),总勃起时间(>10 min)(76/100 vs 88/100,P<0.05),均明显延长;冠状沟平均硬度(≥50%)(78/100 vs 89/100,P<0.05),血清雄激素水平[(137±12)ng/dL vs(165±13)ng/dL,P<0.05]均显著提高.正常男性使用增厚避孕套后的阴茎振动阈值显著升高((50±2.1)级 vs(59±1.8)级,P<0.05);但射精潜伏期及阴茎勃起时间无明显变化(P>0.05).结论 增厚避孕套能够有效延长早泄患者的射精潜伏期,降低阴茎头敏感性,增强阴茎勃起强度,延长阴茎勃起时间,从而提高性交质量,对早泄有一定的治疗作用.而正常男性使用增厚避孕套后的射精潜伏期无明显变化,不推荐正常男性使用增厚避孕套延长射精潜伏期.
Efficacy evaluation of thickened condom in the treatment of premature ejaculation
Objective To assess the efficacy of thickened condoms in the treatment of premature ejaculation.Methods Total of 100 premature ejaculation patients treated at the Department of Urology,First Affiliated Hospital of Zhejiang University School of Medicine,from August to December 2021 were selected as the premature ejaculation group and 30 normal men as the normal group.Safety,comfort,penile dorsal nerve conduction velocity,ejaculation latency,penile sensitivity,erectile function,and serum reproductive hormone levels were evaluated after using both regular and thickened condoms in both groups.Results Compared to regular condoms,premature ejaculation patients using thickened condoms reported lower comfort(80/100 vs.60/100;P<0.05),slower penile dorsal nerve conduction velocity(30.5±2.7)m/s vs.(21.04±1.9)m/s;P<0.05,increased penile vibration threshold(29±2.5)vs.(47±2.0);P<0.05),longer ejaculation latency(>3 min)(16/100 vs.78/100;P<0.05),and prolonged total erectile time(>10 min)(76/100 vs.88/100;P<0.05).Moreover,mean hardness of the coronary sulcus(≥50%)(78/100)vs.(89/100);P<0.05)and serum androgen levels(78±12)ng/dL vs.(89±100)ng/dL;P<0.05 were significantly increased.For normal men using thickened condoms,penile vibration threshold significantly increased(50±2.1)vs.(59±1.8);P<0.05,with no significant changes in ejaculation latency or penile erection time(P<0.05).Conclusion Thickened condoms effectively prolong ejaculation latency in premature ejaculation patients,enhance glans penis sensitivity,improve penile erection strength,and extend erection time,thereby enhancing sexual intercourse quality and offering a therapeutic effect on premature ejaculation.However,thickened condoms do not significantly alter ejaculation latency in normal men,suggesting caution against their use for this purpose.
