Retrospective Analysis of 227 Cases of Pemphigus Complicated with Skin Infection in a Tertiary Hospital of Guangxi
Objective To analyze the situation of pemphigus patients with skin infection,the risk factors of concurrent infection and the influence of concurrent infection on the prognosis of pemphigus in a tertiary hospital in Guangxi.Methods The clinical data of 227 patients with pemphigus hospitalized in the Department of Dermatology and Venereology of the First Affiliated Hospital of Guangxi Medical University from January 2020 to April 2023 were retrospectively analyzed,and the incidence of skin infection and main pathogenic bacteria types were determined.The correlation of infection with underlying diseases and severity of disease and the influence of co-infection on prognosis were analyzed.Results Among 227 patients with pemphigus,174(76.65%)had skin infection.The bacterial smears of 170 patients with erosion and ulcer were positive,and 43 kinds of pathogenic bacteria(28 kinds of bacteria and 13 kinds of fungi)were cultured in 157 patients.Staphylococcus aureus,Pseudomonas aeruginosa and Klebsiella pneumoniae were more common,and 55 cases of candida were cultured.Three patients were infected with viruses.The average length of hospital stay of patients with bacterial infection,multidrug resistance and fungal infection was longer than that of patients without combined infection.Co-infection was associated with age,hypoproteinemia,disease severity and length of hospital stay(P<0.05).Among 227 patients,125 were clinically cured,97 improved,4 relapsed or worsened,and 1 gave up treatment.Conclusion With long summer and hot and humid climate in Guangxi,pemphigus patients have a high incidence of bacterial and fungal infection and occasionally viral infection.Advanced age,hypoproteinemia,severe disease,and past long-term use of glucocorticoids/immunosuppressants are risk factors for co-existing skin infections.The length of hospital stay of co-infected patients increases,which affects the prognosis of patients.
