Classic Kaposi's Sarcoma with Thrombosis:Two Case Report
A 70-year-old male patient presented with purple nodules and plaques on the left foot accompanied by pain for 3 years.Dermatological examination revealed swelling of the left calf and foot,with scattered purple nodules and plaques ranging in size from soybean to egg on the left foot.Laboratory tests confirmed HIV negativity.Histopathological examination of the skin lesion showed hyperkeratosis of the epidermis,acanthosis hyperplasia,and a large number of spindle cells in the dermis arranged in a storiform pattern,interspersed with numerous lumens containing red blood cells.Necrosis was evident in some areas.Immunohistochemical staining revealed scattered positivity for HHV-8,and positivity for CD31 and CD34.Diagnosis of Classic Kaposi's sarcoma of the skin was made.Another 69-year-old male patient reported scattered purple nodules on the left foot with pain for six months.He had a history of venous thrombosis of left lower extremity for 1 year.Dermatological examination showed swelling of the left calf and foot,with several purple nodules ranging in size from green bean to soybean on the left full toe and lateral margin of the left foot,some of which were ruptured and scabbed.HIV testing was negative.Histopathological examination of skin lesion revealed hyperkeratosis of the epidermis,partial hyperplasia of the spinous layer,and nodular infiltration of spindle cells in the dermis.Lumens of varying sizes containing red blood cells were observed.Necrosis was present in some areas.Immunohistochemical staining showed positivity for CD31,CD34,and ERG,with individual positivity for HHV-8.Diagnosis of classic Kaposi's sarcoma of the skin was made.
