三阴性乳腺癌超声特征分析

目的 比较三阴性乳腺癌(triple-negative breast cancer,NTNBC)与非三阴性乳腺癌(non-triple-negative breast cancer,NTNBC)的超声声像特征.方法 收集2015年1月—2022年12月南京医科大学附属逸夫医院收治的乳腺癌患者257例,依据雌激素受体(estrogen receptor,ER)、孕激素受体(progesterone receptor,PR)和人表皮生长因子受体-2(human epidermal growth factor receptor-2,HER-2)表达情况分为NTNBC组196例和TNBC组61例,比较两组患者的超声声像特征.结果 两组患者病灶大小、形态、边界、边缘毛刺、内部回声、内部钙化、后方回声和内部血流分布方面差异有统计学意义(P<0.05).TNBC超声声像形态规则,多为分叶状、圆形或类圆形,形态不规则较少见;肿块较少向周边组织浸润,边界清晰,周边高回声毛刺少见;肿瘤内部回声较低或为混合回声,钙化多为点状微钙化,内部血管形态多呈点状和分支型;后方回声多增强.结论 NTNBC与TNBC在超声声像上存在明显差异,超声可为二者之间的早期鉴别诊断提供依据.
Analysis of Ultrasonic Features of Triple Negative Breast Cancer
Objective To compare the ultrasonographic characteristics of triple-negative breast cancer(TNBC)and non-triple-negative breast cancer(NTNBC).Methods A total of 256 patients with breast cancer who were admitted to Yifu Hospital affiliated to Nanjing Medical University from January 2015 to December 2022 were collected.According to the expression of estrogen receptor(ER),progesterone receptor(PR)and human epidermal growth factor receptor-2(HER-2),they were divided into NTNBC group(196 cases)and TNBC group(61 cases).Results There were significant differences(P<0.05)in the size,shape,boundary,edge spicules,internal echo,internal calcification,posterior echo,and internal blood flow distribution of the lesions between the two groups.TNBC ultrasound images have regular shapes,mostly lobulated,circular,or quasi circular,and irregular shapes are relatively rare.The tumor has less infiltration into surrounding tissues,clear boundaries,and rare high echogenic spicules in the periphery.The internal echo of the tumor is low or mixed,and the calcification is mostly punctate microcalcifications.The internal vascular morphology is often punctate and branching.The posterior echo enhanced mostly.Conclusion There is a significant difference in ultrasound imaging between NTNBC and TNBC,and ultrasound can provide a basis for early differential diagnosis between them.
