摘要
目的 分析肺部超声(lung ultrasound,LUS)评分在新生儿呼吸窘迫综合征(neonatal respiratory distress syndrome,NRDS)病情严重程度及疗效评估中的应用价值.方法 选择2019年1月—2023年1月福州市第一总医院收治的60例NRDS患儿作为NRDS组,根据胸部X线(chest X-ray,CXR)分级标准分为CXRⅠ级21例、CXRⅡ级19例、CXRⅢ级20例.选择同期于我院出生的健康新生儿60例作为对照组.观察研究对象的LUS表现,对不同CXR分级和治疗时间点NRDS患儿的LUS评分进行对比.结果 CXRⅠ级患儿LUS评分低于CXRⅡ级和CXRⅢ级,差异有统计学意义(P＜0.05).相关性分析显示,CXR等级与LUS评分之间具有正相关关系(P＜0.01);治疗前、治疗后4 h、治疗后1 d和治疗后3 d患儿LUS评分差异有统计学意义(P＜0.05).结论 LUS可以为NRDS患儿病情严重程度及疗效评估提供有效的量化评价指标.
Abstract
Objective To analyze the application value of lung ultrasound(LUS)score in the evaluation of severity and efficacy of neonatal respiratory distress syndrome(NRDS).Methods A total of 60 children with NRDS admitted to Fuzhou First General Hospital from January 2019 to January 2023 were selected as the NRDS group.According to the chest X-ray(CXR)classification standard,children with NRDS were divided into CXRⅠ level(21 cases),CXRⅡ level(19 cases),and CXRⅢ level(20 cases).Sixty healthy newborns in our hospital during the same period were selected as the control group.LUS manifestations of the study subjects were observed,and the LUS manifestations of NRDS children with different severity and time points of treatment were compared.Results The LUS score of CXRⅠ level were significantly lower than CXRⅡ level and CXRⅢ level(P<0.05).Spearman correlation analysis showed that there was a positive correlation between CXR grade and LUS score(P<0.01).There were significant differences in LUS scores before treatment and 4 hours after treatment,1 day after treatment and 3 days after treatment(P<0.05).Conclusion LUS can provide an effective quantitative evaluation index for the assessment of severity and efficacy of NRDS children.
