社区多元化护理在老年2型糖尿病患者院外管理中的应用效果

摘要
目的 探讨社区多元化护理对老年糖尿病患者血糖控制、自我管理行为依从性和自我效能的影响.方法 选择2021年10月—2023年10月在天津市北辰区普东街社区卫生服务中心建立健康档案的老年2型糖尿病患者50例,随机分为对照组和管理组各25例.对照组采用常规社区护理管理模式,管理组采用社区多元化护理管理模式.比较两组患者干预3个月后的血糖控制指标、自我管理行为依从性和自我效能.结果 干预3个月后,管理组低血糖发生频次低于对照组,差异有统计学意义(P＜0.05);管理组饮食管理、运动管理、用药管理、足部护理评分高于对照组,差异有统计学意义(P＜0.05);管理组症状管理效能和疾病共性管理效能评分高于对照组,差异有统计学意义(P＜0.05).结论 社区多元化护理能够减少老年2型糖尿病患者的低血糖事件发生,提高其自我管理行为依从性和自我效能.
Abstract
Objective To explore the influence of community diversified nursing on blood glucose control,self-management behavior compliance and self-efficacy of elderly patients with diabetes.Methods From October 2021 to October 2023,a total of 50 elderly patients with type 2 diabetes mellitus(T2DM)who established health records in Pudong Street Community Health Service Center of Beichen District in Tianjin were randomly divided into control group and management group,with 25 cases in each group.The control group adopted a conventional community nursing management model,while the management group adopted a diversified community nursing management model.The blood glucose control indicators,self-management behavior compliance,and self-efficacy were compared between two groups after 3 months of intervention.Results After 3 months of intervention,the frequency of hypoglycemia in the management group was significantly lower than that in the control group(P<0.05).The scores of dietary management,exercise management,medication management,and foot care in the management group were significantly higher than those in the control group(P<0.05).The scores of symptom management efficiency and disease common management efficiency in the management group were significantly higher than those of the control group(P<0.05).Conclusion Community diversified nursing can reduce hypoglycemic events during the treatment process of elderly T2DM patients,and improve their self-management behavior compliance and self-efficacy.
