塞来昔布联合脊柱内镜下经皮椎间孔镜技术治疗老年退行性腰椎间盘突出症的效果观察

摘要
目的 观察塞来昔布联合脊柱内镜下经皮椎间孔镜(transforaminal endoscopic spine system,TESSYS)技术治疗老年退行性腰椎间盘突出症患者的效果.方法 选取2021年3月—2023年2月贵港市港北区人民医院收治的80例老年退行性腰椎间盘突出症患者,随机分为手术组和联合组各40例.手术组采用TESSYS技术进行治疗,联合组在术后加用塞来昔布,比较两组治疗前后的腰椎活动功能、腰背伸肌群生物力学指标、血清炎症指标.结果 术后3个月,联合组视觉模拟评分量表、Oswestry功能障碍指数评分低于手术组,而日本骨科学会评分高于手术组,差异有统计学意义(P<0.05);联合组腰背伸状态下峰力矩、功率高于手术组,而腰背屈伸比低于手术组,差异有统计学意义(P<0.05);联合组白介素-1β、基质金属蛋白酶-3、前列腺素E2水平低于手术组,差异有统计学意义(P<0.05).结论 塞来昔布联合TESSYS技术治疗老年退行性腰椎间盘突出症,可有效缓解疼痛,改善腰椎活动功能和腰背伸肌群生物力学特性,并且降低血清炎症指标.
Abstract
Objective To observe the effect of celecoxib combined with transforaminal endoscopic spine system(TESSYS)technique in the treatment of elderly patients with degenerative lumbar disc herniation.Methods Eighty elderly patients with degenerative lumbar disc herniation admitted to Guigang Gangbei District People's Hospital from March 2021 to February 2023 were randomly divided into operation group and combination group,with 40 cases in each group.The operation group was treated with TESSYS technique,and the combination group was treated with celecoxib capsule after surgery.The lumbar mobility function,biomechanics of lumbar and dorsal extensor muscles and serum inflammatory markers before and after treatment were compared between two groups.Results Three months after surgery,the visual analogue scale and Oswestry dysfunction index scores in the combination group were significantly lower than those in the operation group,while the score of the Japanese Orthopedic Society was significantly higher than that of the operation group(P<0.05).The peak torque and power of the combination group were significantly higher than those of the operation group in the lumbar and dorsal extension state,while the lumbar and dorsal flexion extension ratio was significantly lower than that of the operation group(P<0.05).The levels of Interleukin-1 β,matrix metalloproteinase-3 and prostaglandin E2 in the combinationgroup were significantly lower than those in the operation group(P<0.05).Conclusion The combination of celecoxib and TESSYS technology in the treatment of elderly degenerative lumbar disc herniation can effectively alleviate pain,improve lumbar mobility and biomechanical properties of the lumbar extensor muscle group,and reduce serum inflammatory indicators.
