手术室整体配合在PFNA内固定治疗老年股骨粗隆间骨折中的效果分析

摘要
目的 探讨对行防旋型股骨近端髓内针(proximal femoral nail antirotation,PFNA)内固定治疗的老年股骨粗隆间骨折患者在常规围术期护理期间增加手术室整体配合后的临床效果.方法 选取2020年3月—2022年11月聊城市中医医院收治的接受PFNA内固定治疗的股骨粗隆间骨折的老年患者86例作为研究对象,随机分为对照组和观察组各43例.对照组行常规围术期护理,观察组在对照组基础上加强手术室的整体配合,比较两组患者的临床一般观察指标及护理效果相关指标.结果 观察组的手术时间、术后首次下床活动时间、住院时间及骨折愈合时间均比对照组短,差异均有统计学意义(P＜0.05);观察组术中出血量比对照组少,差异有统计学意义(P＜0.05);术后7 d焦虑、疼痛、膝关节功能改善优于对照组,差异均有统计学意义(P＜0.05).结论 在围绕PFNA内固定术开展常规护理工作期间强化手术室整体配合,可帮助老年股骨粗隆间骨折患者缩短手术治疗及术后康复的时间,同时能更好地减轻焦虑情绪、疼痛程度并改善膝关节功能.
Abstract
Objective To explore the clinical effect of increasing the overall cooperation in the operating room during the routine perioperative care for elderly patients with intertrochanteric femur fractures treated with proximal femoral nail antirotation(PFNA).Methods Eighty-six cases of elderly patients with intertrochanteric femur fractures treated with PFNA internal fixation admitted to Liaocheng City Hospital of Traditional Chinese Medicine from March 2020 to November 2022 were selected as the study subjects,and were divided into 43 cases each in the control group and the observation group by the double-blind method.The control group performed the routine perioperative nursing care,and the observation group strengthened the overall cooperation in the operating room on the basis of the control group.The clinical general indexes and nursing efficacy were compared between two groups.Results The operation time,the first postoperative ambulation,the hospitalization time and the fracture healing time of the observation group were significantly shorter than those of the control group(P<0.05).The intraoperative bleeding in the observation group was significantly less than that of the control group(P<0.05).The postoperative 7 d anxiety,pain,and knee joint healing time of the observation group were significantly shorter than those of the control group(P<0.05).Conclusion Strengthening the overall cooperation in the operating room during the routine nursing work around PFNA internal fixation can help elderly patients with intertrochanteric femur fracture to shorten the time of surgical treatment and postoperative rehabilitation,as well as reduce anxiety,pain level and improve knee joint function.
