氨基葡萄糖联合塞来昔布在老年早期膝关节骨关节炎中的应用

摘要
目的 探讨氨基葡萄糖联合塞来昔布在老年早期膝骨关节炎(knee osteoarthritis,KOA)中的治疗效果.方法 选择2021年1月—2022年12月辽阳市中心医院收治的老年早期KOA患者80例,随机分为对照组和观察组各40例.两组患者均进行基础的生活方式干预,对照组口服塞来昔布,观察组在对照组用药基础上加用氨基葡萄糖,比较两组患者的炎性因子指标和膝关节功能改善情况.结果 治疗后,观察组外周血清肿瘤坏死因子-α、白介素-1β、基质金属蛋白酶-3水平和膝关节液上清可溶性细胞间黏附分子-1、β-连环素水平低于对照组,膝关节液上清骨保护素水平高于对照组,差异有统计学意义(P＜0.05).观察组美国纽约特种外科医院膝关节评分的疼痛、关节活动度、行走功能、关节肌力、稳定性评分及总分高于对照组,差异有统计学意义(P＜0.05).结论 氨基葡萄糖联合塞来昔布治疗老年早期KOA,能够进一步缓解患者外周血和膝关节液中的炎症状态,并且改善膝关节功能.
Abstract
Objective To explore the therapeutic effect of glucosamine combined with celecoxib in the treatment of early stage knee osteoarthritis(KOA)in the elderly.Methods A total of 80 elderly patients with early stage KOA admitted to Liaoyang Central Hospital from January 2021 to December 2022 were randomly divided into a control group and an observation group,with 40 cases in each group.Both groups received basic lifestyle interventions.The control group received oral celecoxib,while the observation group added glucosamine additionally.The inflammatory factor indicators and knee joint function were compared between two groups.Results After treatment,the levels of serum tumor necrosis factor-α,interleukin-1β,matrix metalloproteinase-3 and the levels of knee joint fluid supernatant soluble intracellular adhesion molecule-1,β-catenin in the observation group were significantly lower than those in the control group,while the levels of knee joint fluid supernatant osteoprotegerin in the observation group was significantly higher than that in the control group(P<0.05).The scores of pain,joint range of motion,walking function,joint muscle strength,stability and the total score in the observation group of New York hospital for special surgery knee score were significantly higher than those in the control group(P<0.05).Conclusion The combination of glucosamine and celecoxib can further alleviate the inflammatory status in peripheral blood and knee joint fluid,and improve knee joint function in the treatment of early stage KOA in elderly patients.
