摘要

目的 分析探究颈脑一体化超声在老年急性脑梗死颈动脉狭窄评估中的应用价值.方法 选取2022年12月—2023年12月期在河西学院附属张掖市人民医院进行治疗的老年急性脑梗死患者168例,所有患者均行数字减影血管造影(digital subtraction angiography,DSA)、颈部多普勒超声(carotid doppler ultrasonography,CDU)、经颅多普勒超声(transcranial dopple,TCD)检查.以DSA结果为金标准,将入组患者分为无狭窄组31例与狭窄组137例,并进一步将狭窄组分为狭窄组轻度组46例、中度组58例、重度组33例.比较各组CDU、TCD参数;应用受试者工作特征(receiver operating characteristic,ROC)曲线分析CDU联合TCD对颈动脉狭窄的诊断效能.结果 狭窄组舒张期峰值血流速度、收缩期峰值血流速度、搏动指数、阻力指数均高于无狭窄组,差异有统计学意义(P<0.05);亚组分析显示,上述指标从高到低依次为重度组、中度组和轻度组,差异有统计学意义(P<0.05);ROC曲线分析显示,对颈动脉狭窄及其严重程度的诊断,CDU联合TCD检查的灵敏性和特异性较高,其ROC曲线下面积明显高于单一检查.结论 颈脑一体化超声用于老年急性脑梗死患者,能有效评估颈动脉狭窄严重程度,为临床后续诊治提供指导.

Abstract

Objective To analyze and explore the application value of integrated neck-brain ultrasound to evaluate the carotid artery stenosis in elderly patients with acute cerebral infarction.Methods A total of 168 elderly patients with acute cerebral infarction who underwent treatment in Zhangye People's Hospital Affiliated to Hexi University from December 2022 to December 2023 were selected.All patients underwent digital subtraction angiography(DSA),neck doppler ultrasound(CDU),and transcranial doppler ultrasound(TCD)examinations.The DSA results were used as the gold standard,and they were divided into non-stenosis group(n=31)and stenosis group(n=137).The stenosis group was divided into mild group(n=46),moderate group n=58),and severe group(n=33).The CDU and TCD parameters of each group were compared.The receiver operating characteristic(ROC)curves were applied to analyze the diagnostic efficacy of CDU combined with TCD for carotid artery stenosis.Results The levels of peak diastolic velocity,peak systolic velocity,pulsatility index,and resistance index in the stenosis group were significantly higher than those in the non-stenosis group(P<0.05).Subgroup analysis showed that the above indicators were ranked from high to low as severe group,moderate group,and mild group(P<0.05).ROC curve analysis showed that the sensitivity and specificity of CDU combined with TCD examination were high in the diagnosis of carotid artery stenosis and its severity,and the area under the ROC curve was significantly higher than that of single examination.Conclusion Integrated nec-brain ultrasound can effectively evaluate the severity of carotid artery stenosis and provide guidance for subsequent clinical diagnosis and treatment in elderly patients with acute cerebral infarction.
