院内针对性护理联合延续性护理在白血病PICC置管患儿中的应用价值探讨

摘要
目的 探讨院内针对性护理联合延续性护理在白血病经外周静脉置入中心静脉导管(peripherally inserted central catheter,PICC)置管患儿中的应用价值.方法 本研究选取2022年1月—2023年6月上海市儿童医院收治的80例白血病PICC置管患儿及其主要照护者(80名),随机分为对照组(40例患儿及其陪护者)和延续组(40例患儿及其陪护者),对照组采用常规护理,延续组采用院内针对性护理联合延续性护理,对比分析两组效果.结果 延续组患儿并发症发生率低于对照组,差异有统计学意义(P<0.05).干预前两组照护者心理状况[焦虑自评量表(self-rating anxiety scale,SAS)、抑郁自评量表(self-rating depression scale,SDS)]、护理知识掌握情况评分比较,差异无统计学意义(P>0.05);经干预两组照护者SAS、SDS、护理知识掌握情况评分均得到改善,且延续组各项评分优于对照组,差异均有统计学意义(P<0.05).延续组照护者护理满意度各维度评分高于对照组,差异有统计学意义(P<0.05).结论 在白血病PICC置管患儿及其照护者中应用院内针对性护理联合延续性护理可降低PICC并发症发生率,改善照护者不良情绪、提升照护者知识储备及护理满意度,可临床推广.
Abstract
Objective To explore the application value of in-hospital targeted care combined with continuous care in children with leukemia with peripherally inserted central catheter(PICC)placement.Methods In this study,eighty children with leukemia using PICC catheterization and their primary caregivers(80)admitted to Shanghai Children's Hospital from January 2022 to June 2023 were randomly divided into a control group(40 children and their companions)and a continuity group(40 children and their companions).The control group adopted routine care,while the continuity group adopted in-hospital targeted care combined with continuous care.The effects were compared between two groups.Results The complication rate of children in the continuity group was significantly lower than that of the control group(P<0.05).Before the intervention,there was no significant difference in the scores of psychological condition[self-rating anxiety scale(SAS),depression scale(SDS)]and nursing knowledge mastery between the two groups.After the intervention,SAS,SDS and nursing knowledge mastery between the two groups were all improved significantly(P>0.05).SAS,SDS,and nursing knowledge scores were improved in both groups,and the scores of the continuity group were significantly better than those of the control group(P<0.05).The scores of the dimensions of nursing satisfaction of caregivers in the continuity group were significantly higher than those of the control group(P<0.05).Conclusion The application of in-hospital targeted care combined with continuous care in children with leukemia using PICC and their caregivers can reduce the incidence of PICC complications,improve the caregiver's adverse emotions,enhance the caregiver's knowledge base and satisfaction with care,and can be promoted clinically.
