海马益智散对肾虚痰瘀证轻度血管性认知功能障碍患者海马体积调节作用的研究

摘要
目的 研究海马益智散对肾虚痰瘀证轻度血管性认知功能障碍患者海马体积的调节作用.方法 选择乌鲁木齐市中医医院脑病科2022年1-12月收治的80例肾虚痰瘀证血管性认知障碍患者作为研究对象,随机分为对照组和益智组各40例.对照组采用阿司匹林肠溶片治疗,益智组在对照组基础上加用海马益智散.比较两组治疗前后神经心理测评变化及治疗后海马结构体积.结果 经治疗,益智组精神状态评分、认知功能评分、记忆功能和执行能力评分、日常生活能力评分、缺血情况评分、临床痴呆情况评分均优于对照组,差异均有统计学意义(P＜0.05).治疗后,益智组左侧海马体积(3.24±0.07)cm3、右侧海马体积(3.27±0.13)cm3均大于对照组的(2.51±0.23)cm3、(2.50±0.22)cm3,差异有统计学意义(P＜0.05).结论 海马益智散治疗肾虚痰瘀证轻度血管性认知功能障碍可改善患者的精神状态和认知能力,提高记忆力和执能力,增强日常生活能力,减少出现缺血情况,降低发生痴呆的风险,提高临床疗效.
Abstract
Objective To study the modulating effect of Haima Yizhi Powder on the hippocampal volume of patients with mild vascular cognitive dysfunction with renal deficiency and phlegm stasis evidence.Methods A total of 80 patients with vascular cognitive dysfunction with renal deficiency and phlegm stasis evidence admitted to the Department of Encephalopathy of Urumqi Hospital of Traditional Chinese Medicine from January to December 2022 were selected as the study subjects,and they were randomly divided into 40 cases each in the control group and the puzzle group.The control group was treated with aspirin enteric-coated tablets,and the puzzle group was treated with Haima Yizhi powder on the basis of the control group.The changes in neuropsychological assessment before and after treatment and the volume of hippocampal structure after treatment were compared between the two groups.Results After treatment,the mental state score,cognitive function score,memory function and executive ability score,daily life ability score,ischemic condition score,and clinical dementia condition score of the puzzle group were significantly better than those of the control group(P<0.05).After treatment,the left hippocampus volume(3.24±0.07)cm3 and the right hippocampus volume(3.27±0.13)cm3 of the puzzle group were significantly larger than those of(2.51±0.23)cm3 and(2.50±0.22)cm3 of the control group,respectively(P<0.05).Conclusion Haima Yizhi Powder can improve the mental state and cognitive ability of patients,improve memory and executive ability,enhance the ability of daily life,reduce the occurrence of ischemia,reduce the risk of dementia,and improve the clinical efficacy in the treatment of mild vascular cognitive dysfunction of renal deficiency and phlegm stasis type.
