摘要
目的 分析磁共振成像和肌肉骨骼超声对类风湿膝关节炎的诊断价值.方法 回顾性分析2022年1月—2022年12月福鼎市医院收治的102例疑似类风湿膝关节炎患者的资料,所有均接受磁共振成像和肌肉骨骼超声检查,以最终临床诊断结果为金标准,比较两种检查方法的诊断效能.结果 两种检查方式诊断类风湿膝关节炎的灵敏度、特异度、诊断符合率、阳性预测值、阴性预测值差异无统计学意义(P＞0.05).经一致性分析显示,与最终临床诊断结果比较,磁共振成像诊断关节积液、滑膜增生、骨侵蚀、软骨破坏的Kappa值分别为0.822、0.881、0.869、0.933(P＜0.05);而肌肉骨骼超声诊断关节积液、滑膜增生、骨侵蚀、软骨破坏的Kappa值分别为0.924、0.833、0.788、0.781(P＜0.05).结论 磁共振成像和肌肉骨骼超声在类风湿膝关节炎的诊断中均具有较高的应用价值,二者均可用于疾病诊断和病情监测.
Abstract
Objective To analyze the diagnostic value of magnetic resonance imaging and musculoskeletal ultrasound in rheumatoid knee arthritis.Methods A retrospective analysis was conducted on the data of 102 suspected rheumatoid knee arthritis patients in Fuding City Hospital from January 2022 to December 2022.All patients underwent magnetic resonance imaging and musculoskeletal ultrasound examination,with the final clinical diagnosis as gold standard.The diagnostic efficacy of two methods above were compared.Results There was no statistically significant difference in sensitivity,specificity,diagnostic accuracy,positive predictive value,and negative predictive value between the two examination methods for diagnosing rheumatoid knee arthritis(P>0.05).According to consistency analysis,compared with the final clinical diagnosis results,the Kappa values of magnetic resonance imaging for diagnosing joint effusion,synovial hyperplasia,bone erosion,and cartilage destruction were 0.822,0.881,0.869,and 0.933(P<0.05).The Kappa values for diagnosing joint effusion,synovial hyperplasia,bone erosion,and cartilage damage using musculoskeletal ultrasound were 0.924,0.833,0.788,and 0.781,respectively(P<0.05).Conclusion Magnetic resonance imaging and musculoskeletal ultrasound have high diagnostic value in rheumatoid arthritis,and both can be used for disease diagnosis and monitoring.
