基于心脏超声的冠心病心功能分级与冠状动脉狭窄程度的关系

摘要
目的 分析超声心动图下冠心病患者心功能与冠状动脉(简称冠脉)狭窄程度的关系.方法 纳入河西学院附属张掖市人民医院2021年12月—2023年12月收治的117例冠心病患者为研究对象.所有患者根据超声心动图检查,分为心功能Ⅰ级26例、Ⅱ级40例、Ⅲ级25例、Ⅳ级26例;根据冠状动脉造影检查,分为轻度狭窄38例、中度狭窄45例、重度狭窄34例.对比不同心功能分级、不同冠脉狭窄程度患者的心脏超声指标,分析心功能和冠脉狭窄程度的相关性.结果 随着心功能分级的增加,左室射血分数(left ventricular ejection fraction,LVEF)逐渐降低,左心室收缩末期容积(left ventricular end systolic volume,LVESV)和左室舒张末期容积(left ventricular end diastolic volume,LVEDV)逐渐增加,各组间差异有统计学意义(P＜0.05).相关性分析显示,LVEF与心功能分级呈负相关关系,而LVESV、LVEDV与心功能分级呈正相关关系(P＜0.05).随着冠脉狭窄程度的增加,LVEF逐渐降低,而LVESV和LVEDV逐渐增加,各组间差异有统计学意义(P＜0.05).相关性分析显示,LVEF与冠脉狭窄程度呈负相关关系,而LVESV、LVEDV与冠脉狭窄程度呈正相关(P＜0.05).结论 LVEF、LVESV、LVEDV与冠心病患者心功能分级及冠脉狭窄程度均有相关关系,对患者进行超声心动图检查,有助于对冠心病的诊断及治疗提供准确的依据.
Abstract
Objective To analyze the relationship between heart function grading and the degree of coronary artery stenosis in patients with coronary heart disease based on cardiac ultrasound.Methods One hundred and seventeen patients with coronary heart disease were included in Zhangye People's Hospital Affiliated to Hexi University from December 2021 to December 2023.According to echocardiography examination,there were 26 cases of Grade Ⅰ heart function,40 cases of Grade Ⅱ heart function,25 cases of Grade Ⅲ heart function,and 26 cases of Grade Ⅳ heart function.According to coronary angiography,there were 38 cases of mild stenosis,45 cases of moderate stenosis,and 34 cases of severe stenosis.The cardiac ultrasound indicators of patients with different concentric functional grades and different degrees of coronary stenosis were compared,and their correlation between cardiac function and degree of coronary stenosis were analyzed.Results With the increase of cardiac function grading,left ventricular ejection fraction(LVEF)gradually decreased,left ventricular end systolic volume(LVESV)and left ventricular end diastolic volume(LVEDV)gradually increased(P<0.05).The correlation analysis showed that LVEF was negatively correlated with cardiac function grading,while LVESV,LVEDV were positively correlated with cardiac function grading(P<0.05).With the increases of the degree of coronary artery stenosis,LVEF gradually decreases,while LVESV and LVEDV gradually increase(P<0.05).The correlation analysis showed a negative correlation between LVEF and the degree of coronary stenosis,while LVESV and LVEDV were positively correlated with the degree of coronary stenosis(P<0.05).Conclusion LVEF,LVESV,LVEDV are all related to the cardiac function grading and degree of coronary stenosis in patients with coronary heart disease.Performing echocardiography on patients can provide accurate basis for the diagnosis and treatment of coronary heart disease.
