富血小板纤维蛋白治疗慢性创面的应用价值探讨

摘要
目的 探讨富血小板纤维蛋白(platelet-rich fibrin,PRF)治疗慢性创面的应用价值.方法 选择2023年5-12月南京中医药大学附属医院收治的60例慢性创面患者作为研究对象,按照处理方式的差异分为对照组(清创后泡沫敷料包扎)和观察组(清创后用PRF处理)各30例,对比两组患者创面愈合率、创面愈合时间、感染控制情况(细菌转阴率、白细胞、C-反应蛋白)及疼痛情况.结果 观察组处理后7 d和15 d的创面愈合率高于对照组,差异有统计学意义(P＜0.05);观察组创面愈合时间短于对照组,差异有统计学意义(P＜0.05).观察组处理后7 d和15 d的细菌转阴率高于对照组,差异有统计学意义(P＜0.05).观察组处理后7 d和15 d的白细胞、C-反应蛋白低于对照组,差异有统计学意义(P＜0.05).观察组处理后7 d和15 d的VAS评分低于对照组,差异有统计学意义(P＜0.05).结论 PRF应用于慢性创面的治疗可显著缩短创面愈合时间、降低机体炎性反应水平、提高转阴率的同时减轻了患者的疼痛,值得临床借鉴推广.
Abstract
Objective To explore the application value of platelet-rich fibrin(PRF)in the treatment of chronic wounds.Methods Sixty patients with chronic wounds admitted to the Affiliated Hospital of Nanjing University of Chinese Medicine from May to December 2023 were selected as research subjects,and they were divided into 30 cases in the control group(foam dressing dressing after debridement)and 30 cases in the observation group(PRF treatment after debridement)according to the differences in treatment methods.The two groups were compared in terms of the rate of wounds healing,the time of wounds healing,the control of infection(bacterial turnover rate,white blood cells,C-reactive protein)and pain.Results The wound healing rate of the observation group was higher than that of the control group at 7 and 15 days after treatment,and the difference was statistically significant(P<0.05).The wound healing time of the observation group was shorter than that of the control group,and the difference was statistically significant(P<0.05).The bacterial conversion rate of the observation group after 7 and 15 days of treatment was higher than that of the control group,and the difference was statistically significant(P<0.05).The white blood cells and C-reactive protein levels in the observation group were lower than those in the control group at 7 and 15 days after treatment,and the difference was statistically significant(P<0.05).The VAS scores of the observation group were lower than those of the control group at 7 and 15 days after treatment,and the difference was statistically significant(P<0.05).Conclusion PRF applied to the treatment of chronic wounds can significantly shorten the wound healing time,reduce the level of inflammatory response,improve the conversion rate and reduce the patient's pain at the same time,which is worthy of clinical reference and popularization.
