保乳切除术与全乳切除术治疗乳腺癌的近期效果对比

摘要
目的 对比分析保乳切除术与全乳切除术治疗乳腺癌的近期效果.方法 选取2018年9月—2023年6月在江阴市中医院接受择期保乳切除术治疗的49例乳腺癌患者作为保乳组,选取同期接受择期全乳切除术治疗的49例乳腺癌患者作为对照组.对比分析两组的治疗效果,术后随访9个月的复发率和病死率,以及术前与术后9个月的负性情绪和生活质量.结果 所有患者手术均获得成功,保乳组患者病灶切缘均为阴性.所有患者术后均获得至少9个月的有效随访,两组术后9个月复发率和病死率比较,差异无统计学意义(P＞0.05).保乳组乳房外观优良率高于对照组,差异有统计学意义(P＜0.05).两组术后9个月负性情绪和生活质量评分均优于术前,且保乳组优于对照组,差异有统计学意义(P＜0.05).结论 保乳切除术治疗TNM分期为T1和T2期的乳腺癌患者,近期效果与全乳切除术相当,而且能够提升乳房美观性和患者生活质量,改善患者的不良情绪,值得推广使用.
Abstract
Objective To compare the effects of breast-conserving surgery and total mastectomy in the treatment of breast cancer.Methods A total of 49 cases of patients with breast cancer who underwent breast-conserving surgery in Jiangyin Hospital of Traditional Chinese Medicine from September 2018 to June 2023 were selected as the breast-conserving group,at the same periods,49 patients with breast cancer who underwent total mastectomy were selected as control group.Compare and analyze the treatment effects of the two groups,the recurrence rate and mortality rate after 9 months of postoperative follow-up,as well as the negative emotions and quality of life before and 9 months after surgery.Results All the operations were successful in this study.The surgical margin of breast-conserving group was negative.All patients received effective follow-up for at least 9 months after surgery,and there was no significant difference in the recurrence rate and mortality rate between the two groups at 9 months after surgery(P>0.05).The rate of excellent breast appearance in the breast-conserving group was significantly higher than that in the control group(P<0.05).The negative emotions and quality of life scores of the two groups at 9 months after surgery were significantly better than before treatment,and the breast-conserving group was significantly better than the control group(P<0.05).Conclusion Breast-conserving surgery has the same short-term effect as total mastectomy in the treatment of breast cancer patients with TNM stage T1 and T2,moreover,it can improve breast aesthetics and patient quality of life,alleviate negative emotions,and is worth promoting and using.
