目的 探讨以针刀为主的综合治疗方案治疗后外侧型颈椎间盘突出症的临床疗效.方法 选择 2020 年 1 月—2022 年 1月泉州市正骨医院针灸推拿科收治的82例后外侧型颈椎间盘突出症患者为研究对象,给予针刀结合手法、药物、牵引等综合疗法治疗.记录临床疗效、治疗前后的疼痛情况、颈椎功能障碍情况、生活质量、不良反应.结果 (1)痊愈 20 例(24.2%),显效 36 例(43.9%),有效 19 例(23.2%),无效 7 例(8.5%),总体有效率为 91.5%.(2)治疗前,患者平均视觉模拟评分法(visual analogue scale,VAS)评分为(5.268±0.969)分;治疗后,VAS 评分为(2.317±0.910)分,差异有统计学意义(P＜0.05).(3)治疗前,颈椎功能障碍指数量表(neck disability index,NDI)评分为(434.965±4.182)分;治疗后,NDI 评分为(12.122±4.448)分,差异有统计学意义(P＜0.05).(4)治疗前,患者平均生活质量调查问卷(European organization for research and treatment of cancer quality of life questionnaire core 30 items,QLQ-C30)评分为(50.622±10.426)分;治疗后,QLQ-C30 评分为(82.136±3.915)分,差异有统计学意义(P＜0.05).(5)治疗期间1例患者因针刀对颈椎的刺激而发生血肿,不良反应发生率为1.22%.结论 针刀为主综合治疗后外侧型颈椎间盘突出症的临床疗效肯定,能有效缓解疼痛,改善患者颈椎功能障碍,是治疗此疾病的一种较理想的疗法.
Clinical Efficacy Observation of Comprehensive Treatment With Needle Knife as the Main Method for Posterior Lateral Cervical Disc Herniation
Objective To explore the clinical efficacy of a comprehensive treatment scheme based on acupotomy in the treatment of posterolateral cervical disc herniation.Methods From January 2020 to January 2022,a total of 82 patients with posterolateral cervical disc herniation admitted to department of acupuncture and moxibustion massage,Quanzhou Orthopedic Hospital were selected as the research objects.They were treated with acupotomy combined with manipulation,drugs,traction and other comprehensive therapies.The clinical efficacy,pain before and after treatment,cervical dysfunction,quality of life,and adverse reactions were recorded.Results(1)20 cases(24.2%)were cured,36 cases(43.9%)were significantly improved,19 cases(23.2%)were effective,and 7 cases(8.5%)were ineffective.The overall effective rate was 91.5%.(2)Before treatment,the average visual analog scale(VAS)score of the patient was(5.268±0.969)points.After treatment,the average VAS score of the patients was(2.317±0.910)points,with a statistically significant difference(P<0.05).(3)Before treatment,the patient's average Neck Disability Index(NDI)score was(434.965±4.182)points.After treatment,the average NDI score of the patients was(12.122±4.448)points,with a statistically significant difference(P<0.05).(4)Before treatment,the patient's average quality of life questionnaire(QLQ-C30)score was(50.622±10.426)points.After treatment,the average QLQ-C30 score of the patients was(82.136±3.915)points,with a statistically significant difference(P<0.05).(5)During the treatment period,one patient developed a hematoma due to the stimulation of the cervical spine with a needle knife,with an adverse reaction rate of 1.22%.Conclusion Acupuncture knife based comprehensive treatment of posterolateral cervical disc herniation has a positive clinical effect,which can effectively relieve pain and improve cervical dysfunction,and is an ideal treatment for this disease.
