祛风方治疗尿毒症性皮肤瘙痒的效果

目的 探讨祛风方治疗尿毒症性皮肤瘙痒的临床效果。方法 选取 2021 年 1 月—2023 年 1 月上海中医药大学附属第七人民医院肾病科收治的慢性肾脏病(chronic kidney disease，CKD)4～5 期且符合尿毒症性皮肤瘙痒诊断标准的 100 例患者，随机分为 2 组，各 50 例。治疗组使用祛风方(自拟方+曲安奈德+硫代硫酸钠复方制成)，对照组使用醋酸曲安奈德乳膏。对比 2 组患者治疗前后实验室检查指标，如血白细胞(white blood cells，WBC)、C反应蛋白(C-reactive protein，CRP)、中性粒细胞百分比(N)、白介素-1β(interleukin 1β，IL-1β)、白介素-6(interleukin 6，IL-6)，β2-微球蛋白(β2-microglobulin，β2-MG)等，并比较特应性皮炎评分表(scoring atopic dermatitis，SCORAD)及主要症状疗效率，综合评价疗效。结果 2 组患者治疗后SCORAD评分、主要症状疗效率、实验室检查指标均优于治疗前，且治疗组优于对照组(P＜0。05)。治疗组总有效率为 82。00%，高于对照组的 54。00%，差异有统计学意义(P＜0。05)。结论 治疗组对尿毒症性皮肤瘙痒治疗的有效率优于对照组;此文的研究结果为慢性肾脏病相关性瘙痒症后期标准制定提供了借鉴内容。
Effect of Qufeng Recipe in the Treatment of Uremic Skin Pruritus
Objective To explore the clinical efficacy of Qufeng recipe in the treatment of uremia skin pruritus.Methods A total of 100 patients with stage 4-5 chronic kidney disease(CKD)who met the diagnostic criteria for uremic pruritus admitted to the department of nephrology,Shanghai University of Traditional Chinese Medicine Affiliated Seventh People's Hospital from January 2021 to January 2023 were randomly divided into 2 groups with 50 patients in each group.The treatment group was treated with Qufeng recipe(self-made formula+triamcinolone acetonide+sodium thiosulfate compound),and the control group was treated with triamcinolone acetonide acetate cream.Laboratory test parameters,such as white blood cells(WBC),C-reactive protein(CRP),neutrophil percentage(N),interleukin-1β(IL-1β),interleukin 6(IL-6),β2-microglobulin(β2-MG),etc were compared between the 2 groups before and after treatment,scoring atopic dermatitis(SCORAD)and the efficacy rate of main symptoms were compared.The curative effect was evaluated comprehensively.Results SCORAD score,efficacy rate of main symptoms and laboratory examination indexes of 2 groups after treatment were better than before treatment,and the treatment group was better than the control group(P<0.05).The total effective rate of the treatment group(82.00%)was better than that of the control group(54.00%),and the difference was statistically significant(P<0.05).Conclusion Both groups were effective for uremic pruritus,but the therapeutic effect of the treatment group was better than that of the control group.The research results of this article provide reference content for the formulation of late stage standards for chronic kidney disease related pruritus.
