基于DRG指标的医院学科能力评价体系的构建研究

目的 构建公立医院绩效考核指标和疾病诊断相关分组(diagnosis related groups,DRG)相关指标为基础的医院学科能力评价体系,为医院提高医院资源利用率和运营效率,达到最优性价比提供管理工具。方法 以公立医院绩效考核指标和DRG相关指标为基础,通过查阅与整理文献确定评价指标,结合专家咨询与调查问卷的方式,确定各指标的权重系数,构建医院学科能力评价体系。并利用该体系对福建省某三甲医院进行评价。结果 手术科室中,学科能力评价得分最高的为科室 1,为 85。78 分;科室 2、科室 3 和科室 4 得分也超过 80 分;学科能力评价得分最后的为科室 20。非手术科室中,学科能力评价得分最高的为科室 21,为 96。67 分;科室 22、科室 23、科室 24 和科室 25 得分也超过 85 分;学科能力评价得分最后的为科室 42。诊治疑难危重患者较多、手术量大、手术难度高的科室得分较高,诊治病种单一、操作和手术难度低的科室的得分较低。学科能力评价体系的评价结果基本符合医院临床实际。结论 本研究构建的学科能力评价体系能够准确评价医院的学科能力情况,对医疗机构加强医院学科能力建设和管理有一定的指导意义。
A Study on the Construction of Hospital Discipline Capability Evaluation System Based on DRG Index
Objective To construct a hospital discipline ability evaluation system based on performance evaluation indicators and diagnosis related groups(DRG)-related indicators of public hospitals.Provide management tools for hospitals to improve the utilization rate of hospital resources and operation efficiency and achieve the optimal cost-performance ratio.Methods Based on public hospital performance appraisal and DRG-related indicators,review and sort out the literature,combine expert consultation and questionnaire to determine the evaluation indexes.Determine each index's weight coefficient and construct the hospital discipline ability evaluation system.Then use this system to evaluate a third-class hospital in Fujian province.Results Among surgical departments,department 1 had the highest score of discipline ability evaluation(85.78 points).Department 2,3 and 4 also scored more than 80 points.The last score of subject ability evaluation is department 20.Among the non-surgical departments,the department with the highest score of academic ability evaluation was 21,which was 96.67 points.Departments 22,23,24 and 25 also scored over 85 points.The last score of subject ability evaluation is department 42.The scores of departments which have more challenging and critical patients,large operation volume,and high surgical difficulty were higher,and the scores of departments with single diseases and low operation and surgical complication were lower.The evaluation results of the hospital discipline ability evaluation system basically accord with the clinical practice of the hospital.Conclusion The discipline ability evaluation system established in this study can accurately evaluate the discipline ability of hospitals and has specific guiding significance for strengthening the discipline ability construction and management of medical institutions.
