目的 分析不同程度职业性尘肺疾病合并心血管疾病的危险因素。方法 选取福建省福能集团总医院 2021 年 4 月—2023 年6 月收治的100 例轻中度职业性尘肺疾病合并心血管疾病患者(对照组)与 87 例重度职业性尘肺疾病合并心血管疾病患者(观察组)为对象。比较组间基线资料差异,探讨不同程度职业性尘肺疾病合并心血管疾病的危险因素。结果 单因素分析结果显示,2 组吸烟史、自我抑郁量表(self-depression scale,SDS)评分、高脂血症、超敏C反应蛋白(hypersensitive C-reactive protein,hs-CRP)升高、第 1 秒用力呼气容积(exexhalatory volume in the first second,FEV1)、脉血二氧化碳分压(thermal pressure of CO dioxide,PaCO2)比较,差异有统计学意义(P<0。05);经过logistic回归分析发现,吸烟史、SDS评分、高脂血症、hs-CRP升高、FEV1、PaO2 是重度职业性尘肺疾病合并心血管疾病发生的影响因素(P<0。05)。根据筛选出的影响因素建立重度职业性尘肺疾病合并心血管疾病影响因素预测模型,logit(P)= ln(P/1-P)=10。341-1。573X吸烟史-0。669X高脂血症+0。626XSDS评分+0。732Xhs-CRP-0。686XFEV1+0。710XPaO2。根据模型拟合程度检验Hosmer-Lemeshow检验分析可得,模型拟合效果较好(χ2 = 3。112,P<0。05)。结论 吸烟史、SDS评分、高脂血症、hs-CRP升高、FEV1、PaO2 是重度职业性尘肺疾病合并心血管疾病发生的影响因素,临床在对职业性尘肺疾病合并心血管疾病患者治疗中,应根据相关危险因素给予患者针对性防护,改善患者预后。
Analysis of Risk Factors for Different Degrees of Occupational Pneumoconiosis Disease Combined With Cardiovascular Disease
Objective To analyze the risk factors of different degrees of occupational pneumoconiosis disease combined with cardiovascular disease.Methods A total of 100 patients(control group)with mild to moderate occupational pneumoconiosis disease combined with cardiovascular disease and 87 patients(observation group)with severe occupational pneumoconiosis disease combined with cardiovascular disease admitted to Fujian Funeng Group General Hospital from April 2021 to June 2023 were selected as the subjects.The baseline data differences between the groups were compared to explore the risk factors of different degrees of occupational pneumoconiosis disease combined with cardiovascular disease.Results Univariate analysis showed that there were statistically significant differences in smoking history,self-depression scale(SDS)score,hyperlipidemia,elevated hypersensitive C-reactive protein(hs-CRP),exexhalatory volume in the first second(FEV1)and thermal pressure of CO dioxide(PaO2)between two group(P<0.05).After logistic regression analysis,it was found that smoking history,SDS score,hyperlipidemia,elevated hs-CRP,FEV1,and PaO2 were the influencing factors for the occurrence of severe occupational pneumoconiosis disease combined with cardiovascular disease(P<0.05).Based on the selected influencing factors,a predictive model for the influencing factors of severe occupational pneumoconiosis disease combined with cardiovascular disease was established,logit(P)=ln(P/1-P)=10.341-1.573 Xsmoking history-0.6 6 9 X hyperlipidemia+ 0.6 2 6 X SDS score+ 0.7 3 2 X hs-CRP-0.686 XFEV1+0.710 XPaO2.According to the model fitting degree test and the Hosmer-Lemeshow test analysis,it can be concluded that,the model fitting effect is good(χ2=3.112,P<0.05).Conclusion Smoking history,SDS score,hyperlipidemia,elevated hs-CRP,FEV1,and PaO2 are influencing factors for the occurrence of severe occupational pneumoconiosis disease with cardiovascular disease.In clinical treatment of patients with occupational pneumoconiosis disease with cardiovascular disease,targeted protection should be given according to relevant risk factors to improve patient prognosis.
