Application Value of HPV Detection Combined With TCT in the Diagnosis of Cervical Intraepithelial Neoplasia
Objective To analyze the value of human papillomavirus(HPV)detection combined with thinprep cytologic test(TCT)in clinical diagnosis of cervical intraepithelial neoplasia(CIN).Methods A total of 416 women who underwent CIN screening in department of clinical laboratory, the Second Affiliated Hospital of Xiamen Medical College from January to December 2021 were selected, all of whom underwent HPV and TCT tests.Among them,150 were diagnosed as CIN by cervical tissue pathology and 266 were non CIN.Using cervical tissue pathology as the gold standard, the value of combining HPV and TCT tests for clinical diagnosis of CIN was analyzed. Results The Kappa value of the combined diagnosis and cervical histopathological diagnosis was 0.877,which is higher than the Kappa value of 0.749 in HPV detection and cervical histopathological diagnosis, and also higher than the Kappa value of 0.694 in TCT examination and cervical histopathological diagnosis. The sensitivity, specificity, accuracy, positive predictive value, and negative predictive value of the combination of HPV and TCT in the diagnosis of cervical cancer were 96.00%(144/150),93.23%(248/266),94.23%(392/416),88.89%(144/162),and 97.64%(248/254),respectively,higher than those of HPV[87.33%(131/150),88.72%(236/266),88.22%(367/416),81.37%(131/161),92.55%(236/255)]and TCT alone[88.67%(133/150),83.46%(222/266),85.34%(355/416),75.14%(133/177),92.89%(222/239)],with statistically significant differences(P<0.05).Conclusion Both HPV detection and TCT examination have certain application value in clinical diagnosis of CIN,and the combination of the two can enhance the diagnostic value.
