目的 探讨血清miR-221 在慢性心力衰竭中的表达水平及与心肌重塑、心功能的关系与临床意义。方法 选取聊城市第四人民医院 2020 年 3 月—2022 年 2 月收治的 110 例慢性心力衰竭患者作为研究对象。按照纽约心脏病协会(New York Heart Association,NYHA)分级标准将该组患者分为Ⅰ~Ⅱ级组(n= 38)、Ⅲ级组(n= 35)及Ⅳ级组(n= 37),全部患者均接受了彩色多普勒超声检查诊断,同时选择来聊城市第四人民医院进行彩色多普勒超声检查的 30 名健康人作为对照组。同时测量血清miR-221 的表达水平,对比不同心功能分级下慢性心力衰竭患者血清miR-221 的表达水平以及心功能指标的差异性,之后采用Pearson相关性分析探讨血清miR-221 表达水平与心功能指标的相关性。结果 Ⅳ级组血清miR-221 表达水平为(3。29±0。59),高于Ⅲ级组的(2。31±0。10)、Ⅰ~Ⅱ级组的(1。12±0。09)及对照组的(0。54±0。21),Ⅲ级组血清miR-221 表达水平高于Ⅰ~Ⅱ级组及对照组,Ⅰ~Ⅱ级组血清miR-221 表达水平高于对照组,差异有统计学意义(P<0。05)。Ⅳ级组左室舒张末内径(left ventricular end-diastolic diameter,L V E D D)、左心房内径(l e f t atrial diameter,LAP)、左心室后壁厚度(left ventricular posterior wall thickness,LVPW)、左室质量指数(left ventricular mass index,LVMI)高于Ⅲ级组、Ⅰ~Ⅱ级组及对照组,左室重构指数(left ventricular remodeling index,LVRI)、心排出量(cardiac output,CO)、左室射血分数(left ventricular ejection fraction,LVEF)低于Ⅲ级组、Ⅰ~Ⅱ级组及对照组;Ⅲ级组LEVDD、LAP、LVPW、LVMI高于Ⅰ~Ⅱ级组及对照组,LVRI、CO、LVEF值低于Ⅰ~Ⅱ级组及对照组;Ⅰ~Ⅱ级组LEVDD、LAP、LVPW、LVMI高于对照组,LVRI、CO、LVEF低于对照组,差异有统计学意义(P<0。05)。Pearson相关性分析显示,血清miR-221 与LVEDD、LAP、LVPW、LVRI分别呈正相关性,与LVRI、CO及LVEF呈负相关性(P<0。05)。结论 血清miR-221 在慢性心力衰竭中具有较高的表达水平,且这种表达水平的升高与心肌重构以及心脏功能之间具有密切的相关性。
Expression of Serum miR-221 in Chronic Heart Failure and Its Relationship With Myocardial Remodeling and Cardiac Function
Objective To explore and analyze the expression level of serum miR-221 in chronic heart failure and its relationship with myocardial remodeling and cardiac function and its clinical significance.Methods A total of 110 patients with chronic heart failure admitted to Liaocheng Fourth People's Hospital from March 2020 to February 2022 were selected as research objects.According to the New York Heart Association(NYHA)grading standard,these patients were divided into grade Ⅰ to Ⅱ group(n=38),grade Ⅲgroup(n=35)and grade Ⅳ group(n=37).All patients were diagnosed by color Doppler ultrasonography.At the same time,30 healthy people who came to Liaocheng Fourth People's Hospital for color Doppler ultrasound examination were selected as the control group.Meanwhile,the expression level of serum miR-221 was measured,and the differences of serum miR-221 expression level and cardiac function indexes in patients with chronic heart failure under different cardiac function grades were compared.After that,Pearson correlation analysis was used to investigate the correlation between the expression level of serum miR-221 and cardiac function indexes.Results The expression level of serum miR-221 in gradeⅣgroup was(3.29±0.59),higher than that in gradeⅢ group(2.31±0.10),grade Ⅰ to Ⅱ group(1.12±0.09)and control group(0.54±0.21),the expression level of serum miR-221 in gradeⅢ group was higher than that in grade Ⅰ to Ⅱ group and control group,and the expression level of miR-221 in grade Ⅰ to Ⅱ was higher than that in control group,the differences were statistically significant(P<0.05).The left ventricular end-diastolic diameter(LVEDD),left atrial diameter(LAP),left ventricular posterior wall thickness(LVPW)and left ventricular mass index(LVMI)of gradeⅣ group were higher than those of gradeⅢ group,grade Ⅰ toⅡ group and control group,the left ventricular remodeling index(LVRI),cardiac output(CO),left ventricular ejection fraction(LVEF)were lower than that of gradeⅢ group,grade Ⅰ toⅡ group and control group.LEVDD,LAP,LVPW and LVMI of grade Ⅲ group were higher than those of grade Ⅰ to Ⅱ group and control group,and LVRI,CO and LVEF values were lower than those of grade Ⅰ to Ⅱ group and control group.LEVDD,LAP,LVPW and LVMI in grade Ⅰ toⅡgroup were higher than those in control group,and LVRI,CO and LVEF were lower than those in control group,the differences were statistically significant(P<0.05).Pearson correlation analysis showed that serum miR-221 was positively correlated with LVEDD,LAP,LVPW and LVRI,respectively,and negatively correlated with LVRI,CO and LVEF,respectively(P<0.05).Conclusion Serum miR-221 has a high expression level in chronic heart failure,and the increase of this expression level is closely correlated with myocardial remodeling and cardiac function.
