Prognostic Predictive Value of PCT,sTREM-1 and HMGB1 in Patients With Severe Pneumonia Combined With Respiratory Failure
Objective To explore high mobility group box 1(HMGB1),prognosticative value of receptor-1(sTREM-1)and procalcitonin(PCT)in patients with severe pneumonia complicated with respiratory failure.Methods A total of 78 patients with severe pneumonia combined with respiratory failure admitted to Yanbian University Hospital from April 2020 to April 2023 were divided into treatment ineffective group(n=44)and treatment effective group(n=34)according to their treatment conditions.General data,PCT,sTREM-1 and HMGB1 levels of the two groups of patients were observed,and ROC curves of PCT,sTREM-1,HMGB1 and their combination were plotted.The relationship between the three serum indexes and their combination and the prognosis of the disease was analyzed.Results The respiratory rate,mean arterial pressure,acute physiological and chronic health scores,sequential organ failure scores and CRP levels in the ineffective group were compared with those in the effective group,and the differences were significant(P<0.05).The PCT,sTREM-1 and HMGB1 in the ineffective treatment group were higher than those in the effective treatment group,and the differences were significant(P<0.05).The combined AUC value of the three indexes was 0.958,95%CI 0.920-0.995,sensitivity was 97.70%,specificity was 71.50%,and cut-off value was 0.008.Conclusion The combined test of PCT,sTREM-1 and HMGB1 can predict the prognosis of patients with severe pneumonia complicated with respiratory failure,and its sensitivity and specificity are high.
