目的 探讨冠心病(coronary heart disease,CHD)患者血清转化生长因子-β1(transforming growth factor-β1,TGF-β1)、氨基末端B型脑钠肽前体(nitrogen terminal B type natriuretic peptide precursor,NT-proBNP)水平对冠状动脉病变严重程度的影响。方法 选取 2019 年 7 月—2022 年 6月在山东省潍坊市第二人民医院心内科诊治的 85 例CHD患者作为观察组,同期健康体检者 39 名作为对照组。均采用双抗体夹心酶联免疫吸附试验(enzyme linked immunosorbent assay,ELISA)法检测TGF-β1;采用电化学发光法检测NT-proBNP。比较 2 组患者生化指标、血清TGF-β1、NT-proBNP水平,以及观察组冠状动脉病变不同程度时血清TGF-β1、NT-proBNP水平,分析评估血清TGF-β1、NT-proBNP诊断CHD的价值。结果 2组三酰甘油(triacylglycerol,TG)、总胆固醇(total cholesterol,TC)、高密度脂蛋白胆固醇(high density lipoprotein-cholesterol,HDL-C)、低 密 度脂蛋白胆固醇(low density lipoprotein-cholesterol,LDL-C)比较,差异无统计学意义(P>0。05)。观察组TGF-β1 为(326。04±181。75)ng/L,低于对照组的(926。42±156。47)ng/L,观察组血清NT-proBNP为(134。94±22。16)pg/mL,高于对照组的(65。25±3。35)pg/mL,差异有统计学意义(P<0。01)。单支血管病变血清TGF-β1 高于两支血管病变、多支血管病变,差异有统计学意义(P<0。01);单支血管病变NT-proBNP低于两支血管病变、多支血管病变,差异有统计学意义(P<0。01)。结论 观察组血清TGF-β1 水平降低,NT-proBN水平升高,与冠状动脉病变严重程度密切相关,两者联合检测可作为CHD早期诊断、病情评估的检测指标。
Changes of Serum TGF-β1,NT-proBNP in Patients With Coronary Heart Disease and Its Impact on Coronary Stenosis
Objective To investigate the effects of serum transforming growth factor-β1(TGF-β1)and nitrogen terminal B type natriuretic peptide precursor(NT-proBNP)levels on the severity of coronary artery disease in patients with coronary heart disease(CHD).Methods A total of 85 CHD patients diagnosed and treated in the department of cardiology,Weifang NO.2 Peoples Hospital,Shandong Province from July 2019 to June 2022 were selected as the observation group,and 39 healthy physical examination subjects were selected as the control group during the same period.TGF-β1 was detected by double antibody sandwich enzyme linked immunosorbent assay(ELISA).NT-proBNP was detected by electrochemical luminescence method.Biochemical indicators,serum levels of TGF-β1 and NT-proBNP were compared between the two groups,and serum levels of TGF-β1 and NT-proBNP in different degrees of coronary artery lesions in the observation group,and the value of serum TGF-β1 and NT-proBNP in diagnosing CHD was analyzed and evaluated.Results There was no significant difference in triacylglycerol(TG),total cholesterol(TC),high density lipoprotein-cholesterol(HDL-C),and low density lipoprotein-cholesterol(LDL-C)between the two groups(P>0.05).TGF-β1 in the observation group was(326.04±181.75)ng/L,lower than that in the control group(926.42±156.47)ng/L,and serum NT-proBNP in the observation group was(134.94±22.16)pg/mL,higher than that in the control group(65.25±3.35)pg/mL.The difference was statistically significant(P<0.01).The serum TGF-β1 in single-vessel disease was higher than that in two-vessel disease and multi-vessel disease,and the difference was statistically significant(P<0.01).The NT-proBNP in single-vessel disease was lower than that in two-vessel disease and multi-vessel disease,and the difference was statistically significant(P<0.01).Conclusion The serum TGF-β1 level and NT-proBN level increased in the observation group,strongly correlated with the severity of coronary artery lesions,the combined detection of the two can be used as a detection index for the early diagnosis and evaluation of coronary heart disease.
