Effect of Percutaneous Posterior Transforaminal Spinal Nucleectomy for the Treatment of Lumbar Intervertebral Disc Herniation
Objective To explore the application effect of percutaneous transforaminal endoscopic nucleus pulposus removal through lateral posterior approach in the treatment of lumbar disc herniation.Methods A total of 91 patients with lumbar disc herniation at Mindong Hospital Affiliated to Fujian Medical University from January 2017 to December 2019 were selected as the control group.Posterior approach fenestration of the intervertebral disc nucleus pulposus was performed for treatment,and 91 patients with lumbar disc herniation at Mindong Hospital Affiliated to Fujian Medical University from January 2020 to December 2022 were selected as the observation group.Lateral posterior approach percutaneous foraminal endoscopic nucleus pulposus removal was performed for treatment,and the therapeutic effects were compared,including intraoperative bleeding volume surgical time,length of hospital stay,Japanese Orthopaedic Association scores(JOA),postoperative visual analog scale(VAS),and incidence of complications.Results The intraoperative blood loss in the observation group was less than that in the control group,the operation time was longer than that in the control group,but the postoperative hospitalization time was shorter than that in the control group,with statistical significance(P<0.05).After operation,the JOA score of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).The VAS score of observation group was lower than that of control group 1 and 3 days after operation(P<0.05).The total complication rate of the observation group was 5.49%,which was lower than that of the control group(15.39%)(P<0.05).Conclusion Posterior percutaneous foramen endoscopic nucleus pulposectomy has high application value in the treatment of lumbar disc herniation.It can not only reduce overall treatment time and intraoperative bleeding,but also improve lumbar function and alleviate pain.In addition,this method helps to reduce the incidence of postoperative complications in patients.
