Development and Clinical Application of Analgesic Safety Check Table for Intraspinal Labor
Objective To develop a safety checklist for intraspinal labor analgesia and explore its clinical application. To establish the safe implementation strategy of intraspinal labor analgesia to ensure the safety of mother and child. Methods A total of 1 072 parturients who received intraspinal labor analgesia in Zhongda Hospital of Southeast University from January 2020 to December 2021 were selected as the observation group, and 850 parturients who received intraspinal labor analgesia from January 2018 to December 2019 were selected as the control group for retrospective comparison.Control group taked on the labor analgesia routine operation, observation group in the basis of the conventional process and application in Zhongda Hospital of Southeast University for labor analgesia intra-spinal canal safety check list to inspect each link of labor analgesia intra-spinal canal. The incidence of common complications of intraspinal labor analgesia and maternal delivery outcomes were compared between the two groups, the incidence of adverse neonatal outcomes was compared between the two groups, and the executive ability of medical staff in each link of intraspinal labor analgesia was compared between the two groups. Results The incidence of common anesthesia complications was compared between the two groups. The incidence of intrapartum fever was 11.18%in the control group and 8.21%in the observation group. The incidence of urinary retention was 10.94%in control group and 6.25%in observation group. The difference was statistically significant(P<0.05).The cesarean section rate was 9.53%in the control group and 6.06%in the observation group. The natural delivery rate was 89.65%in the control group and 93.47%in the observation group. The difference was statistically significant(P<0.05).The total satisfaction of the control group was 98.71%and the observation group was 99.81%.The difference was statistically significant(P<0.05).There was statistical significance in the occurrence of fetal distress between the two groups(P<0.05).There was statistical significance in the standardization of operation procedures before and after intervention(P<0.05).Conclusion The safety checklist for intraspinal labor analgesia can effectively reduce the rate of cesarean section and the incidence of maternal and child complications in our hospital, improve the standardization of medical staff in the implementation of each link of intraspinal labor analgesia, populalize natural childbirth under analgesic conditions, promote the construction of modern delivery rooms in China, and improve the happiness and satisfaction of pregnant women and their families. It is worthy of clinical promotion.
