Practice of Clinical Pharmacists in Implementing Standardized Management of Inpatients With Hypertension
Hypertension is a common chronic disease.If it is not treated in time and effectively,it can damage the structure and function of important organs such as heart,brain and kidney.The clinical complications caused by hypertension not only cause disability and high mortality,but also consume a lot of medical and social resources,causing a huge burden to families and society.Hypertensive patients usually need a long time of standardized treatment to achieve the purpose of controlling blood pressure,delaying and reducing target organ damage and clinical complications.However,because it is difficult for patients to obtain scientific,systematic and standardized pharmaceutical care,such patients have insufficient knowledge of disease and drug treatment.Irrational drug use is common,and there are a lot of misunderstandings,resulting in poor compliance with drug treatment,and ultimately unable to achieve the goal of lowering blood pressure,affecting the overall treatment effect.Based on the above reasons,the author studied and summarized the standardized management of hospitalized patients with hypertension,aiming to explore the establishment of a pharmaceutical service model for patients with hypertension,and provide reference for clinical pharmacists to carry out chronic disease management in the future.
