我国口腔颌面外科护士开展面神经功能训练的现状调查研究

目的 了解我国口腔颌面外科护士对周围性面瘫患者开展面神经功能训练的现状,分析其影响因素,为面神经功能训练规范的制订提供依据.方法 2023 年 5 月,采用整群抽样法对全国 47 家医院的 612 名口腔颌面外科护士进行调查,并分析其影响因素.结果 收到 612 份有效问卷.知识培训维度:54.74%(335/612)护士接受过相关培训;知识条目回答的正确率为 2.29%~29.08%;多元线性回归分析结果显示,科室学习、医院培训、文献自学、继续教育是护士培训效果的正向影响因素.护士态度维度:99.02%(606/612)的护士赞同开展训练,综合性医院护士的态度条目得分均高于专科医院,2组比较差异有统计学意义(P<0.05);多元线性回归分析结果显示,患者的知情同意、医护的临床判断、护士接受的培训、训练的临床疗效是护士开展态度的正向影响因素.临床应用维度:93.62%(44/47)的医院有收治面瘫患者;61.70%(29/47)的医院已开展训练,均为三级医院;在训练方法方面,开展辅助训练、主动训练及抗阻训练的护士占比分别是 74.18%(454/612)、75.00%(459/612)及 61.44%(376/612);多重响应分析结果显示,护士开展训练主要依据是诊疗指南及专家共识、面神经功能训练相关教材、技术规范、行业标准及团体标准,其累计百分比为 67.90%.结论 口腔颌面外科护士认可面神经功能训练的疗效,开展训练态度积极,但接受培训的机会较少,缺乏相关培训及临床实践指引,导致训练方法繁多、水平参差不齐、护士相关知识水平薄弱.因此,亟须形成统一的面神经功能训练规范,为临床的培训和实践提供指导依据.
Investigation on the Current Situation of Facial Nerve Function Training Conducted by Oral and Maxillofacial Surgery Nurses in China
Objective To understand the current situation of facial nerve function training for peripheral facial paralysis patients,analyze the influencing factors,and provide a basis for the formulation of facial nerve function training norms.Methods In May 2023,a total of 612 oral and maxillofacial surgery nurses in 47 hospitals nationwide were investigated and the influencing factors were analyzed.Results A total of 612 valid questionnaires were received.The knowledge training dimension showed that 54.74%(335/612)of the nurses had received relevant training.The correct rate of answers to knowledge items ranged from 2.29%to 29.08%.Multiple linear regression analysis revealed that department learning,hospital training,literature self-study,and continuing education were positive factors affecting the effectiveness of nurse training.In terms of nurses'attitude dimension,99.02%(606/612)of the nurses agreed to the training.The attitude scores of general hospital nurses were higher than those of specialized hospitals,and the difference between the two groups was statistically significant(P<0.05).The results of multiple linear regression analysis indicated that patients'informed consent and clinical efficacy were positive influencing factors of nurses'attitude.Regarding the clinical application dimension,93.62%(44/47)of the hospitals admitted patients with facial paralysis,and 61.70%(29/47)of the hospitals provided training.All were tertiary hospitals.In terms of training methods,the proportion of nurses conducting auxiliary training,active training,and resistance training was 74.18%(454/612),75%(459/612),and 61.44%(376/612)respectively.The results of the multiple-response analysis showed that the training of nurses was primarily based on diagnosis and treatment guidelines,expert consensus,textbooks related to facial nerve function training,technical specifications,industry standards,and group standards.Its cumulative percentage was 67.90%.Conclusion Oral and maxillofacial surgery nurses recognize the efficacy of facial nerve function training and have a positive attitude towards training,but they have few opportunities to receive training,lack of relevant training and clinical practice guidance,resulting in various training methods,uneven level and weak knowledge level of nurses.Therefore,it is urgent to form a unified training standard of facial nerve function to provide guidance basis for clinical training and practice.
