Practice and Exploration of Paying According to Disease Point Method in TCM Dominant Diseases in a Tertiary General Hospital in Liuzhou Under DRG Payment
Objective To analyze the practice of a tertiary general hospital in Liuzhou City in implementing the point payment for dominant diseases of Traditional Chinese Medicine under DRG payment framework,so as to provide reference for optimizing hospital payment management and improving the reform scheme.Methods Cases of TCM dominant diseases paid by disease point method and cases of the same diagnosis paid by DRG in a tertiary general hospital in Liuzhou City from July 2022 to December 2023 were taken as samples.SPSS 24.0 statistical software were used for analysis and descriptive statistics.The number of payment declaration and passing cases of TCM dominant disease points method,the average cost and length of hospital stay between the two payment methods of TCM for lumbar disc herniation,the profit and loss of 2 payment methods of TCM treatment of lumbar disc herniation were compared in a third-grade general hospital in Liuzhou City.Results From July 2022 to December 2023,the number of declared cases,declared diseases,declared departments and the number of successfully settled cases of TCM dominant diseases per month showed an increasing trend.Non-surgical cases treated by Chinese medicine for"lumbar disc herniation"were selected and divided into 2 groups according to DRG payment and TCM dominant disease payment.Comparison of average cost between the 2 groups showed no statistical significance(P>0.05).The number of days in hospital in the TCM dominant disease paid group was(8.81±2.72)d,and the proportion of balance cases was 60.42%,which was higher than that of the DRG paid group(6.77±2.86)d and 34.62%(P<0.05).Conclusion Tertiary general hospitals should make full use of the opportunities arising from the reform of the payment method for Chinese medicine services,develop Chinese medicine departments and Chinese medicine treatment programs,and nurture talents in Chinese medicine,so as to comply with the reform of the diversified payment method.It is recommended that the Ministry of Health Insurance improve the selection and admission criteria for dominant diseases of TCM,strengthen the information construction and timely feedback of data on payment for dominant diseases of TCM,to further improve the refined management of payment for dominant diseases of TCM.
