Establishment and Effect Research of Hospital Clinical Pathway Under the Integration of DRG and DIP
Objective To explore the method of establishing hospital clinical pathway under the fusion diagnosis related groups(DRG)and diagnosis-intervention packet(DIP),and analyze its actual effect.Methods The patients with appendicitis who underwent laparoscopic appendectomy from March 2021 to June 2023 in the Second Affiliated Hospital of Shandong First Medical University were collected.The observation group included 268 medical records and the control group included 270 medical records.The clinical pathway of appendicitis in the control group was enrolled,and the observation group implemented the integrated management path under the integration of DRG and DIP.Then the enrollment situation in the two groups was counted,and the average drug,consumables and total hospitalization expenses,as well as the hospitalization time and medical insurance balance expenses of the two groups were compared,and the changes of time and cost consumption index of the two groups were analyzed.Results The number of cases enrolled in the clinical pathway was 263 in the observation group,it was higher than that 217 cases of control group,and the completion rate of clinical pathway was 98.13%,it was higher than 80.37%of control group(P<0.05).The average drug cost,consumables cost and total hospitalization cost in the observation group were significantly lower than those in the control group(P<0.05).The total hospitalization time in the observation group was lower than that in the control group,and the medical insurance balance cost of diseases was higher than control group(P<0.05).The consumption index and expense consumption index in the observation group were both lower than control group(P<0.05).Conclusion Clinical pathway management under the integration of DRG and DIP can improve the enrollment rate of clinical pathway,reduce medical expenses,shorten the total hospitalization time and reduce the medical insurance payment.
