A Comparative Study of the Efficacy and Prognosis of Laparoscopic Hysterectomy Combined With Sacral Ligament Suspension and Transvaginal Hysterectomy in the Treatment of Moderate to Severe Uterine Prolapse
Objective To compare the treatment effects and prognosis of laparoscopic total hysterectomy combined with high suspension of the sacral ligament versus transvaginal total hysterectomy for moderate to severe uterine prolapse(UP).Methods A total of 63 patients with moderate and severe UP who were treated in Rushan People's Hospital from January 2018 to March 2022 were selected,and the patients were divided into observation group(n=31)and control group(n=32)according to the difference of selected surgical methods.The observation group adopted laparoscopic hysterectomy combined with high uterosacral ligament suspension,and the control group adopted transvaginal hysterectomy.The treatment effects of the two groups were compared.Results The intraoperative blood loss in the observation group was(33.1±5.6)mL,which was lower than(73.6±4.4)mL in the control group(P<0.05).In the observation group,the time of postoperative indwelling catheter was(3.2±1.2)d,the time of operation was(58.2±6.5)min,and the time of hospitalization was(6.9±1.5)d,which was shorter than(6.1±1.3)d,(70.4±10.3)min,(8.6±2.3)d of the control group(P<0.05).The PFIQ-7 score of the observation group was(21.0±4.4)points after 6 months,which was lower than(28.6±5.1)points of the control group(P<0.05).The total effective rate of the observation group was 100%,higher than 81.3%of the control group,and the difference was statistically significant(P<0.05).Conclusion Compared with transvaginal hysterectomy,laparoscopic hysterectomy combined with high suspension of the sacral ligament has a more significant therapeutic effect on moderate to severe UP,and its application in clinical practice should be strengthened.
