Risk Factors and Prevention Strategies of Neurosurgical Catheter-Associated Urinary Tract Infection
Objective To explore the risk factors of neurosurgical catheter-associated urinary tract infection(CAUTI)and the prevention and intervention strategies.Methods A total of 218 patients with indwelling catheter admitted to the department of neurosurgery,the First Affiliated Hospital of Guangxi Medical University from January 2018 to October 2023 were selected as the study objects.The incidence of CAUTI in patients with different clinical characteristics was analyzed.Multivariate logistic regression analysis was carried out and corresponding preventive intervention strategies were proposed.Results Among 218 patients with neurosurgical catheter,CAUTI occurred in 35 patients(16.06%).Among the 35 CAUTI patients,103 strains of pathogenic bacteria were detected,82 strains of Escherichia coli(79.61%),19 strains of Klebsiella pneumoniae(18.45%)and 2 strains of Staphylococcus aureus(1.94%).There were statistically significant differences in urinary tract infection rates among different gender,age,consciousness disorder,frequency of urethral orifice cleaning,catheter retention time,catheter replacement time,24 h urine volume,history of previous major surgery,and different numbers of other diseases(P<0.05).The results of multivariate logistic regression analysis showed that gender was female,age>65 to 75 years old,patients with consciousness disorder,frequency of urethral opening cleaning once/d,catheter retention time>10 d,catheter replacement time>15 d were the risk factors for high urinary tract infection(P<0.05).Conclusion The incidence of CAUTI is high in patients with indurating catheter in neurosurgery.It is necessary to focus on the clinical observation of female patients,aged>65 to 75 years,patients with consciousness disturbance,urethral opening cleaning frequency once/d,catheter induration time>10 d,catheter replacement time>15 d,and formulate targeted preventive measures to reduce the risk of CAUTI occurrence.
