基于ITHBC理论的健康教育在心脏瓣膜置换中的应用效果

目的 分析正中开胸心脏瓣膜置换患者应用基于健康行为改变整合理论(integrated theory of health behavior change,ITHBC)健康教育的效果。方法 选取 2022 年 7 月—2023 年 6月广西壮族自治区人民医院收治的正中开胸心脏瓣膜置换患者120 例,将其根据不同健康教育方式分为对照组(60 例,常规健康教育)与观察组(60 例,常规健康教育+基于ITHBC理论的健康教育)。2 组均干预 4 个月。比较 2 组护理的依从性、满意度(干预后),干预前后心理状态、生活质量、自我管理能力。结果 观察组各项指标依从率(合理饮食 96。67%、定期复诊 95。00%、作息规律 96。67%、情绪稳定 98。33%、按医嘱用药 96。67%)均高于对照组(合理饮食 86。67%、定期复诊83。33%、作息规律 85。00%、情绪稳定 86。67%、按医嘱用药86。67%)(P<0。05)。干预后,观察组满意度高于对照组(P<0。05)。相较于干预前,2 组干预后的焦虑自评量表(self-rating anxiety scale,SAS)、抑郁自评量表(self-rating depression scale,SDS)评分、生活质量各项评分降低,且组间进行对比,观察组较低(P<0。05)。干预后,2 组自我管理能力各项评分均升高,且观察组较对照组低(P<0。05)。结论 正中开胸心脏瓣膜置换患者应用基于ITHBC理论的健康教育,可有助于提高患者依从性,改善生活质量,增强自我管理能力,消除不良情绪,进而获得患者认可,且有助于为临床改善正中开胸心脏瓣膜置换患者预后提供参考及依据。
The Application Effect of Health Education Based on ITHBC Theory in Heart Valve Replacement
Objective To analyze the effect of health education based on integrated theory of health behavior change(ITHBC)theory in patients undergoing heart valve replacement with median thoracotomy.Methods From July 2022 to June 2023,a total of 120 patients with median thoracotomy heart valve replacement admitted to the People's Hospital of Guangxi Zhuang Autonomous Region were selected.According to the grouping method of different health education methods,they were divided into the control group(60 cases,routine health education)and the observation group(60 cases,routine health education+health education based on ITHBC theory).Two groups were intervened for 4 months.The nursing related indexes of the two groups:compliance,satisfaction(after intervention),psychological state,quality of life and self-management ability before and after intervention were compared.Results The compliance rate of each index of the observation group(reasonable diet 96.67%,regular follow-up 95.00%,regular schedule 96.67%,emotional stability 98.33%,medication according to doctor 's advice 96.67%)were higher than that of the control group(reasonable diet 86.67%,regular follow-up 83.33%,regular schedule 85.00%,emotional stability 86.67%,medication according to doctor 's advice 86.67%)(P<0.05).After intervention,the satisfaction of the observation group was higher than that of the control group(P<0.05).Compared with before intervention,the scores of self-rating anxiety scale(SAS),self-rating depression scale(SDS)and quality of life of the two groups decreased after intervention,and the scores of the observation group were lower than those of the control group(P<0.05).After intervention,the scores of self-management ability of the two groups increased,and those of the observation group were higher than the control group(P<0.05).Conclusion The application of health education based on ITHBC theory in patients with median thoracotomy heart valve replacement can help improve patient compliance,improve quality of life,enhance self-management ability,eliminate bad emotions,and then obtain patient recognition.It is helpful to provide reference and basis for improving the prognosis of patients with median thoracotomy heart valve replacement.
