血清炎症因子水平与肺癌合并肺部感染患者的相关性研究

目的 探讨血清降钙素原(procalcitonin,PCT)、C反应蛋白(C-reactive protein,CRP)、高迁移率族蛋白B1(recombinant high mobility group protein 1,HMGB1)水平、白细胞计数(white blood cell,WBC)与肺癌合并肺部感染患者的关系,为临床治疗提供参考。方法 选取 2021 年 1 月—2022 年 12 月广西壮族自治区民族医院收治的 159 例患者为对象。其中肺癌未合并肺部感染者 95 例作为对照组,肺癌合并肺部感染者 64 例作为观察组。根据肺部感染评分(clinical pulmonary infection score,CPIS)将其分为轻度组(n=22)、中度组(n=21)、重度组(n=21)。分析肺癌合并肺部感染者病原菌分布,比较 2 组炎症因子水平、通气功能,不同程度肺部感染患者炎症因子水平、通气功能,并分析其相关性。结果 64 例肺癌合并肺部感染者痰液标本共检出 69 株病原菌,其中有 18 株革兰阳性菌、42 株革兰阴性菌、9 株真菌;观察组CRP、PCT、HMGB1、WBC、气道峰压(peak inspiratory pressure,PIP)、呼吸做功(work of breath,WOB)、响应频率(frequency response,Fres)高于对照组,动态顺应性(dynamic pulmonary compliance,Cdyn)低于对照组,差异有统计学意义(P<0。05);轻度组、中度组、重度组血清CRP、PCT、HMGB1、WBC、PIP、WOB、Fres呈依次升高趋势,差异有统计学意义(P<0。05);轻度组、中度组、重度组Cdyn呈依次降低趋势,差异有统计学意义(P<0。05);血清PCT、WBC水平与感染程度呈正相关(r=0。363、0。382,P<0。05)。结论 肺癌合并肺部感染患者病原菌主要以革兰阴性菌为主,与肺癌患者相比,其血清炎症因子水平更高,通气功能指标较差,且其水平变化与感染程度存在一定关系,临床上可对相关指标进行监测,为临床诊治肺癌合并肺部感染提供参考依据。
Study on the Correlation Between Serum Inflammatory Factor Level and Lung Cancer Patients With Pulmonary Infection
Objective To explore the relationship between patent cooperation treaty(PCT),C-reactive protein(CRP),recombinant high mobility group protein 1(HMGB1),white blood cell(WBC),and lung cancer patients with pulmonary infection,and provide reference for clinical treatment.Methods A total of 159 patients admitted to the Minzu Hospital of Guangxi Zhuang Autonomous Region from January 2021 to December 2022 were selected as the subjects.Among them,95 cases of lung cancer without lung infection were used as control group,and 64 cases of lung cancer with lung infection were used as observation group.According to the clinical pulmonary infection score(CPIS),the patients were divided into mild group(n=22),moderate group(n=21),and severe group(n=21).The distribution of pathogenic bacteria in lung cancer patients with pulmonary infection was analyzed,the levels of inflammatory factors and ventilation function of the two groups were compared,and the levels of inflammatory factors and ventilation function of patients with different degrees of pulmonary infection were analyzed,and the correlation was analyzed.Results A total of 69 strains of pathogenic bacteria were detected in sputum samples of 64 patients with lung cancer,including 18 Gram-positive bacteria,42 Gram-negative bacteria and 9 Fungi.The levels of CRP,PCT,HMGB1,WBC,peak inspiratory pressure(PIP),work of breath(WOB)and frequency response(Fres)in observation group were higher than those in control group,and dynamic pulmonary compliance(Cdyn)was lower than that in control group,the difference was statistically significant(P<0.05).Serum CRP,PCT,HMGB1,WBC,PIP,WOB and Fres in mild,moderate and severe groups were increased successively,and the difference was statistically significant(P<0.05).Cdyn in mild group,moderate group and severe group was decreased successively,the difference was statistically significant(P<0.05).Serum PCT and WBC levels were positively correlated with the degree of infection(r=0.363,0.382,P<0.05).Conclusion Gram-negative bacteria are the main pathogens in patients with lung cancer complicated with lung infection.Compared with patients with lung cancer,the levels of serum inflammatory factors are higher,and the indicators of ventilation function are worse,and the changes in their levels are related to the degree of infection.Clinical monitoring of relevant indicators can provide a reference for clinical diagnosis and treatment of lung cancer complicated with lung infection.
