危急值精细化管理模式的构建及应用效果分析

目的 探讨以问题和需求为导向构建危急值精细化管理模式的应用效果。方法 回顾性分析 2022 年 7 月—2023 年7 月山东省枣庄市立医院出现危急值的住院患者 1 623 例,将2022 年 7-12 月山东省枣庄市立医院为基线调查阶段,作为对照组(n=810),本阶段医院危急值管理采用传统的粗放式管理模式。将 2023 年 1-7 月为实施干预措施阶段,作为干预组(n=813),本阶段医院危急值管理采用以问题和需求为导向构建危急值精细化模式。观察 2组在危急值通报及时率、危急值登记完整率、正确率,科室危急值及时处理率,病历中危急值记录合格率,危急值管理合格率等方面有无差异性。结果 干预组在危急值通报及时率 100%,对照组为 94。69%,2 组比较差异无统计学意义(P<0。05)。干预组在危急值登记完整率为 97。91%、正确率为 96。82%,对照组危急值登记完整率为 93。33%、正确率为 93。34%,2 组比较差异有统计学意义(P<0。05)。干预组科室危急值及时处理率为 100%,病历中危急值记录合格率为 98。03%和危急值管理合格率为97。67%;对照组科室危急值及时处理率为 89。63%,病历中危急值记录合格率为 89。97%和危急值管理合格率为 89。63%,2 组比较差异有统计学意义(P<0。001)。结论 以问题和需求为导向构建危急值精细化管理体系的应用,打破传统管理模式的弊端,规范危急值管理行为,提高临床危急值管理效率。
Analysis of the Construction and Application Effect of the Refined Management Model of Critical Value
Objective To explore the application effect of constructing a refined management model of critical value oriented by problems and needs.Methods A total of 1623 hospitalized patients with critical values at Zaozhuang Municipal Hospital,Shandong Province from July 2022 to July 2023 were Retrospective analysed.From July to December 2022,Zaozhuang Municipal Hospital,Shandong Province was selected as the baseline investigation stage.As the control group(n=810),the traditional extensive management mode was adopted for critical value management in this stage.The period from January to July 2023 is taken as the stage of implementing intervention measures.As the intervention group(n=813),the critical value management of hospitals in this stage adopts the problem-and demand-oriented critical value refinement model.The timely rate of critical value reporting,the complete rate of critical value registration and the correct rate of the two groups were observed.Department emergency value timely treatment rate;Qualified rate of critical value records in medical records;There is no difference in the qualified rate of critical value management.Results The timely reporting rate was 100%in the intervention group and 94.69%in the control group.There was no significant difference between the two groups(P<0.05).In the intervention group,the completeness rate of critical value registration was 97.91%and the correct rate was 96.82%,while in the control group,the completeness rate of critical value registration was 93.33%and the correct rate was 92.34%,and the difference between the two groups was statistically significant(P<0.05).In the intervention group,the timely handling rate of critical value was 100%,the qualified rate of critical value recording was 98.03%and the qualified rate of critical value management was 97.67%.The timely handling rate of emergency value in control group was 89.63%,the qualified rate of emergency value recording in medical records was 89.97%,and the qualified rate of emergency value management was 89.63%,there was statistical significance between two groups(P<0.001).Conclusion The application of problem-and demand-oriented construction of the refined management system of critical value breaks the drawbacks of the traditional management mode,standardizes the behavior of critical value management,and improves the efficiency of clinical critical value management.
