目的 分析糖尿病自身免疫抗体联合检测在糖尿病分型中的应用价值。方法 将 2021 年 1 月—2023 年 11 月于东莞市中医院进行诊治的 64 例糖尿病患者纳为研究组,其中 1 型糖尿病(type 1 diabetes mellitus,T1DM)患者 22 例,2 型糖尿病(type 2 diabetes mellitus,T2DM)患者 42 例。同期体检健康者 61 例纳为对照组,2 组均接受糖尿病自身免疫抗体检测,比较 2 组的抗体差异性,以及其在糖尿病分型中的价值。结果 研究组血清谷氨酸脱羧酶抗体(glutamic acid decarboxylase antibody,GADA)、胰岛细胞抗体(islet cell antibody,ICA)、胰岛素自身抗体(insulin autoantibodies,IAA)以及联合检测阳性率高于对照组,差异有统计学意义(P<0。05);T1DM患者GADA、ICA、IAA以及联合检测阳性率高于T2DM患者,差异有统计学意义(P<0。05);T1DM、T2DM诊断中联合检测的应用价值高于单一抗体检测。结论 糖尿病自身免疫抗体联合检测有助于临床更好地鉴别糖尿病分型,为后续治疗提供有价值的指导。
Application Value of Combined Detection of Diabetic Autoimmune Antibodies in the Classification of Diabetes Mellitus
Objective To analyze the application value of combined detection of diabetic autoimmune antibodies in diabetes classification.Methods A total of 64 diabetic patients diagnosed and treated in Dongguan Hospital of Traditional Chinese Medicine from January 2021 to November 2023 were included in the study group,incluing 22 patients with type 1 diabetes mellitus(T1DM),42 patients with type 2 diabetes mellitus(T2DM)and 61 healthy patients were included in the control group during the same period.Both two groups were tested for diabetes classification antibodies.The difference of the antibody between the two group and its value in diabetes classification were compered.Results The positive rates of serum glutamic acid decarboxylase antibody(GADA),islet cell antibody(ICA),insulin autoantibodies(IAA)and their combined detection in the study group were higher than those in the control group,and the differences were statistically significant(P<0.05).The positive rates of ICA,IAA and the combined test of T1DM patients were higher than those of T2DM patients,and the difference were statistically significant(P<0.05),and the application value of the combined test in the diagnosis of T1DM,T2DM was higher than that of single antibody test.Conclusion The combined detection of diabetic autoimmune antibodies can help to better identify the types of diabetes mellitus in clinical practice and provide valuable guidance for subsequent treatment.
