摘要

目的 分析风险评估联合计划-执行-检查-处理(plan-do-check-act,PDCA)循环法在多重耐药菌感染(multiple drug-resistant organism,MDRO)防控中的应用效果.方法 选取 2021 年 7 月—2023 年 6 月菏泽市中医医院重症加强护理病房(intensive care unit,ICU)收治的患者 360 例,按照干预时间将 2021 年 7 月—2022 年 6 月的 180 例患者作为对照组,2022 年 7 月—2023 年 6 月的 180 例患者作为研究组.对照组采取常规管理措施防控MDRO,研究组在其基础上采用风险评估联合PDCA循环法进行MDRO防控.比较 2 组MDRO防控措施的执行情况与MDRO的检出率.结果 研究组的危急值报告、规范隔离、开具隔离医嘱、诊疗用品专用、诊疗时个人防护、手卫生、终末消毒的执行情况评分均高于对照组,差异有统计学意义(P<0.05).研究组MDRO检出率为5.56%,低于对照组的 12.22%(P<0.05).结论 风险评估与PDCA循环法联合应用可以进一步提升MDRO防控工作的执行情况,降低MDRO发生风险.

Abstract

Objective To analyze the application effect of the risk assessment combined plan-do-check-act(PDCA)cycle method in the prevention and control of multidrug-resistant Organism(MDRO).Methods A total of 360 patients admitted to the intensive care unit(ICU)of Heze Hospital of Traditional Chinese Medicine from July 2021 to June 2023 were selected.According to the intervention time,180 patients from July 2021 to June 2022 were selected as the control group,and 180 patients from July 2022 to June 2023 were selected as the study group.The control group adopted routine management measures to prevent and control MDRO,while the research group used risk assessment combined with PDCA cycle method for MDRO prevention and control based on this.The implementation of two groups of MDRO prevention and control measures with the detection rate of MDRO were compared.Results The study group scored higher than the control group in terms of critical value reporting,standardized isolation,issuance of isolation medical orders,specialized diagnostic and treatment equipment,personal protection during diagnosis and treatment,hand hygiene,and final disinfection,the differences were statistically significant(P<0.05).The detection rate of MDRO in the research group was 5.56%,which was lower than 12.22%of the control group(P<0.05).Conclusion The combination of risk assessment and PDCA cycle method can effectively improve the implementation of MDRO prevention and control measures,reduce the risk of MDRO occurrence.
