Analysis of Ultrasound Characteristics and Diagnostic Value of Musculoskeletal Ultrasound Examination in Gouty Arthritis
Objective To investigate the ultrasonographic characteristics and diagnostic value of musculoskeletal ultrasound(MSUS)in gouty arthritis.Methods A total of 80 patients with joint pain or swelling of limbs who received musculoskeletal ultrasonography in the Second Affiliated Hospital of Xiamen Medical College from May 2021 to April 2023 were selected and divided into gout group(gouty arthritis,n=40)and control group(non-gouty inflammatory joint disease,n=40)according to clinical diagnosis results.The differences in the affected parts of joints,the features and differences of MSUS imaging between the two groups were compared,the diagnostic efficacy of MSUS in gouty arthritis were evaluated,and the difference of MSUS imaging between the elevated uric acid group and the normal uric acid group in gout group was compared.Results A total of 86 joints were involved in 40 patients in the gout group.A total of 71 joints were involved in 40 patients in the control group.The first metatarsophalangeal joint and ankle joint in the gout group were more involved than those in the control group,the difference was statistically significant(P<0.05).The finger joint in control group was more involved than that in gout group,the difference was statistically significant(P<0.05).In the gout group,38,44,13 spots of double track sign,strong or high echo point and gouty stone were significantly higher than 0,2,0 spots in the control group,the difference was statistically significant(P<0.05).In the gout group,including 38 double-track signs,44 strong or high echo points,13 gouty stones,and 27 cases with at least one positive among the three.In the control group,including 0 double-track signs,2 strong or high echo points,0 gouty stones,and 2 cases with at least one positive among the three.In MSUS examination,the diagnostic specificity and sensitivity of the combination of double track sign,strong or high echo spot and gouty stone were 95.00%and 67.50%.Conclusion Joint lesions in gouty arthritis patients are more likely to involve the first metatarsophalangeal joint and ankle joint.MSUS examination can detect a variety of ultrasonographic features in the affected joints of gouty arthritis patients.Among them,the combination of two track sign,strong or high echo spot and gouty stone has high specificity and sensitivity in the diagnosis of gouty arthritis,and is not affected by short-term fluctuations in blood uric acid level.It is valuable for the diagnosis of gouty arthritis.
